摘要
目的:通过改善头颈肩面膜的制作方法,解决由于面膜收缩给患者带来的不适,同时验证其对放疗位置精度的影响。方法:将需进行头颈肩面膜制作的患者随机分成两组。每组10人,A组患者常规制作面膜,行CT扫描定位;B组患者在制作面膜时在头枕下加垫1.5mm塑料片,取下塑料片后行CT扫描定位。两组病人常规设计放疗计划并行位置验证,分别对比BEV及验证片位置误差。结果:A组患者的X、Y、Z三个方向的摆位误差分别为:2.45±0.59mm,2.64±0.46mm,1.15±0.17mm;B组患者的X、Y、Z三个方向的摆位误差分别为:2.25±0.55mm,2.4±0.40mm,1.17±0.11mm。A、B两组内X、Y、Z做One-way ANOVA分析,其F值分别为12.88和15.31,P值小于0.05,A、B两组内的X、Y、Z有统计学意义。A、B两组间对应X、Y、Z配对t检验,P>0.05,差别无统计学差异。结论:通过头枕下加垫片的方式减少了热塑面膜收缩引起的患者不适,并未降低摆位精度,是一种可行有效的方法。
Objective Through improving the method of making head and shoulder mask, we want to solve the problem that patients often feel discomfort due to the shrinkage of mask, and to verify the effect of position accuracy in radiotherapy. Methods: The patients who need to make head and shoulder mask were randomly divided into two groups. Each group has 10 patients , In the group A , The conventional method are used to make head and shoulder mask and CT scanning; In the group B, 1.5mm plastic sheet is put under the head pillow pad when patients are making mask, and taken off when patients are scanning CT. Then patients in each group are designed conventional radiotherapy planning and verifying position by comparing BEV and iVIEW GT respectively. Results: In the group A the setup error in X, Y, Z were: 2.45 ± 0. 59mm, 2. 64 ± 0. 46mm, 1.15 ± 0. 17mm; in the group B the setup error in X, Y, Z were: 2. 25 ±0. 55mm, 2. 4 ±0.40mm, 1.17 ±0. 11mm. In each group data of the setup error in X, Y, Z were analyzed by One -way ANOVA , the F values were 12. 88 and 15.31 respectively, P 〈0. 05, In each groups the setup error in X, Y, Z had statistical significance. Between A and B group the setup error in X, Y, Z were analyzed by paired t test, P 〉 0.05, no statistical significance difference was found between The three direction respectively. Conclusion: 1.5mm plastic sheet is put under the head pillow pad when patients are making mask and taken off when patients are scanning CT. This is a new approach to make mask. It can reduce the shrinkage of the thermoplastic mask and discomfort of patients. The accuracy of setup is unaffected. The new approach is an effec- tive and feasible method.
