摘要 目的 :探讨高原性肺水肿X线影像特点及其诊断价值。材料和方法 :分析 5 4例高原性肺水肿患者发病 2天内胸片特点。结果 :(1)肺内改变有毛玻璃状、斑点状、小片状影、肺门区块状影、弥漫性实变等五种类型 ,常混合出现 ,可单侧或双侧 ,以右侧略多 ;(2 )心脏改变 :右室轻度增大、心腰略丰满 ;(3)肺门改变 :中心分布为主者肺门影模糊 ,增大不明显 ;肺野分布为主者 ,肺门影正常 ;(4)胸膜改变 :常伴有水平裂增厚。 4~ 5天复查 ,上述改变完全消失。结论 :X线胸片的影像特征对诊断高原性肺水肿有一定的临床价值。 Purpose: To evaluate X ray characteristics and diagnostic value of high altitude pulmonary edema (HAPE). Material and Methods: The chest film of 54 cases with HAPE was analyzed within three days of onset. Results: There were five patterns of pulmonary changes of HAPE. Two or more patterns may mixed in one case and also in one or both sides, but predominately in the right side. The X ray features were ground glass attenuation in hilar and nearby pulmonary prenchyma, spot attenuation, patchy attenuation, hilar mass, diffuse consolidation. Cardiac changes included right ventricular slightly enlargement, pulmonary artery trunck enlarged. The hilar shadow may undelineate in pulmonary edema with predominantly central distribution, but it may be showed when the hilar edema mainly in periphery distribution. Pleura changes were minor fissure thickened. After treatment, all the aboves disappeared about 4~5 days. Conclusion: HAPE were pulmonary alveolus edema. Chest film is useful in the diagnosis of HAPE.
