可吸收挤压螺钉与Endobutton固定在重建前交叉韧带中的比较研究 被引量:3

Comparative study on anterior cruciate ligament reconstruction between bioabsorbable interference screws and endobutton fixation

摘要 [目的]比较研究前交叉韧带(ACL)重建时,关节镜下采用可吸收挤压螺钉与Endobutton固定股骨侧止点的疗效。[方法]我院自2008年7月-2012年6月对35例前交叉韧带断裂患者进行重建,16例采用可吸收挤压螺钉固定,19例采用Endobutton固。术前、术后进行以日常活动、膝关节疼痛、肿胀等情况观察,以及Lachman试验、轴移试验评估膝关节的稳定性,用Lysholm评分法评价膝关节功能,Tegner评分评价运动水平。[结果]本组35例患者均获得6~48个月的随访。挤压螺钉系统组:随访中6例膝关节活动时仍有轻度疼痛,Lachman征Ⅰ度阳性;2例关节疼痛明显,Lachman征Ⅱ度阳性。Endobutton组:随访中3例,膝关节活动时仍有轻度疼痛,无严重疼痛者,4例患者Lachman征Ⅰ度阳性。两组患者中无关节内感染发生。两组术前、术后Lysholm膝关节功能评分及Tegner运动水平评分比较(P〈0.05),两者功能比较差异有统计学意义。[结论]前交叉韧带重建时,可吸收挤压螺钉与Endobutton均可使股骨侧达到初始稳定。Endobutton系统骨隧道扩大效应较挤压螺钉固定明显,但Endobutton技术,操作更单,固定更持久,远期疗效良好,更值得推广。 [Objective]To compare the effect of recoustruct anterior cruciate ligament (ACL), with absorbable screw fixation and with endobutton femoral attachment point under arthroscopy. [ Method ] A total of 35 cases of anterior cruciate ligament rupture patients were admitted in our hospital in July 2008 to June 2012 for reconstruction, 16 cases with bioabsorbable interference screw fixation, 19 cases by Endobutton. Preoperative and postoperative Daily activities, knee pain and swelling of knee gaint were, observed. Stability of knee joint was evaluation by Lachman test and pivot shift test, function of knee joint was evaluation by Lysholm score and Tegner score was used to evaluated the level of exercise of knee joint. Follow-up was made for 6-48 month. [ Results]Extrusion screw system: follow-up was made in 6 cases the knee joint activities still had mild pain syndrome, Lachman I degree positive in 2 cases of joint pain significantly, Lachman syndrome II degree positive. Group Endobutton: follow-up in 3 cases, knee joint activities was still mild pain, no severe pain, 4 cases of patients with Lachman syndrome were one-degree positive. No joint infection occurred in two patients. Comparison of the Lysholm knee score and Tegner activity score of two groups of preoperative, postoperative (P 〈 0.05), there was significant difference between the two functions. [Conclusion ] In anterior cruciate ligament reconstruction, bioabsorbable interference screws and Endobutton can make the femoral side reach initial stability. The system bone tunnel enlargement in more effective in endobutton effect than the extrusion screw fixation, but the technology of endobutton and the operation is more simple, more durable and the long-term effect is good and worthy of promotion.
