摘要
正常的动作电位依赖于多个离子通道的精确调控,以维持心肌细胞的电生理特性.近来发现生理状态下晚钠电流(INaL)可持续存在于动作电位的复极相,幅度小于峰钠电流的1%,却对动作电位的形态(平台期)及时程(QT间期)有着很大的影响,INaL升高1%~3%即可产生病理性改变.心力衰竭时钠离子通道失活减慢且密度上调,钠离子持续内流致动作电位时程延长,诱发心律失常.该文将就INaL在心力衰竭及相关心律失常中的作用和由此引发的治疗新思路予以综述.
The normal action potential(AP) depends on the precise regulation of multiple ion channels, which maintain the electrical physiological characteristics of myocardial cells. Recently late sodium current (INaL) has been found to exist consistently in the repolarization phase under physiological conditions, while its amplitude is less than 1% of peak sodium current, INaL has obvious effect on the formation of the shape ( platform period) and the duration ( QT interphase) of AP, pathologic changes may occur when INaL rises 1%-3%. Heart failure makes the devitalization of sodium channel slower and the density of sodium channel upregulated,continuous inflow of sodium ion leads to the prolongation of action potential duration, and induces a serious of arrhythmias. Here is to make a summary of the role of INaL in the heart failure and related arrhythmias, and the novel treatment strategy of heart failure focusing on INaL.
