经皮激光椎间盘减压术治疗椎间盘源性腰痛的疗效评价

摘要 目的:探讨经皮激光椎间盘减压术( PLDD)治疗椎间盘源性腰痛的疗效。方法回顾分析2011年6月~2013年6月临床诊断为椎间盘源性腰痛患者共28例。术后随访3~24个月,平均12.5个月,比较术前与末次随访疼痛视觉模拟评分、功能障碍评分变化,按改良MacNab功能评价标准评价患者对治疗的主观满意度。结果与术前相比,术后VAS评分降低(3.7±1.3),差异有显著性(P<0.01),术后ODI评分降低(13.7±5.0),差异有显著性(P<0.01);无神经根损伤、大出血、死亡病例,显效6例,有效17例,无效5例,满意度82.1%.结论经皮激光椎间盘减压术为治疗椎间盘源性腰痛安全、有效的方法,值得推广。 Objective To evaluate the efficacy of percutaneous laser disc decompression treatment on disco-genic low back pain .Methods Retrospective analysis including 28 cases of patients with a clinical diagnosed discogenic low back pain from June 2011 to June 2013 was performed .Patients were followed up 3~24 months ,an average of 12 .5 months,and the score changes of VAS ,ODI were observed preoperative and final follow-up.According to the modified MacNab criteria ,satisfaction of the treatment was evaluated .Results Compared with the preoperative ,Postoperative VAS score decreased(3.7 ±1.3),and there were significant differences (P〈0.01).Postoperative ODI score decreased (13.7 ±5.0),there were significant differences (P〈0.01).No nerve root injury,bleeding or deaths appeared ,there were 6 cases being significantly effective in treatment ,17 cases being effective ,5 cases being ineffective ,and the satisfaction was 82.1%.Conclusion Percutaneous laser disc decompression is a safe and effectivefor method for the treatment of disco -genic low back pain .
