摘要
目的比较高频电刀与低温等离子对儿童扁桃体组织的热损伤。方法选取67例需行双侧扁桃体切除术的患儿为研究对象,以随机方法对一侧扁桃体采用高频电刀凝切术切除,另一侧扁桃体采用低温等离子刀切除。按照手术方式将其分为高频电刀组与低温等离子刀组,取两组扁桃体标本的深面将其包埋、切片,HE染色后,在光学显微镜下用图像分析软件IPP测量扁桃体热损伤深度,并对两组数据进行统计分析。结果低温等离子刀组切割时对扁桃体深面热损伤深度为(0.48±0.27)mm,高频电刀组一般凝切时对扁桃体深面热损伤深度为(1.23±0.24)mm,两组差异有统计学意义(P<0.05)。低温等离子刀组与高频电刀组在凝血时对扁桃体深面热损伤深度分别为(1.42±0.31)mm和(1.37±0.26)mm,两组差异无统计学意义(P>0.05)。结论低温等离子刀对扁桃体深面热损伤小于高频电刀,但在凝血时,低温等离子刀与高频电刀对扁桃体深面热损伤无明显差别。
Objective To compare the thermal injury caused by high frequency electrotome and low-temperature plasma in tonsillectomy for children. Methods 67 children patients who needed to received bilateral tonsillectomy were selected as research subjects,and randomly received high frequency electrocoagulation excision for one side of the tonsil, and low-temperature plasma excision for the other side.The patients were allocated to a high frequency electrotome group and a low-temperature plasma group according to different surgery types.Deep layers of tonsil specimens from the two groups were embedded,sectioned and dyed by HE,and image analysis software,IPP,was applied to measure the depth of thermal injury in tonsils under an optical microscope.Data from the two groups were then collected and analyzed. Results The depth of thermal injury in deep layers of tonsils was (0.48±0.27) mm in the low-temperature plasma group,and that in the high frequency eleetrotome group was (1.23±0.24) mm during electrocoagulation.The difference of the data from the two groups was statistically significant (P〈0.05).The depths of thermal injury in deep layers of tonsils during blood coagulation in the low-temperature plasma group and the high frequency electrotome group were (1.42± 0.31) mm and (1.37±0.26) mm respectively,and the difference was not statistically significant (P〈0.05). Conclusion The thermal injury of deep layers of tonsils caused by low-temperature plasma cutter is minor than that caused by high frequency electrotome,whereas the thermal injury caused by the two methods during blood coagulation has no obvious difference.
