谷氨酰胺对急性坏死性胰腺炎大鼠肠道粘膜局部免疫功能的保护作用 被引量:10

摘要 目的 观察急性坏死性胰腺炎时肠粘膜局部免疫功能的变化 ,探讨谷氨酰胺对肠粘膜的保护作用。方法  5 4只SD大鼠随机分成三组①假手术组 (SO ,n =18)②胰腺炎组 (ANP ,n =18)③谷氨酰胺组 (GLN ,n =18) ,通过中心静脉插管输注TPN液 ,谷氨酰胺组添加 3%力肽 (相当于 2 %谷氨酰胺 ) ,三组营养液均等氮。术后 2 4、48、72小时处死动物 ,留取标本 ,分别用于测定肠内容物sIgA含量、肠内容物细菌sIgA包被率、肠粘膜组织中CD+3 、CD+4 、CD+8和k+、λ+细胞数量。结果 在 2 4、48、72小时 ,ANP组、GLN组和SO组的肠内容物细菌sIgA包被率分别为 5 0 %、33%、33%、87%、10 0 %、10 0 %和 10 0 %、10 0 %、10 0 % ,ANP组较SO组差异明显(P <0 0 5 ) ,而GLN组与SO组间无显著差异 (P >0 0 5 )。肠内容物中sIgA的含量在ANP组、GLN组、SO组分别为 15± 9pg/g、13±10pg/g、9± 9pg/g ;2 3± 11pg/g、19± 14pg/g、2 0± 12pg/g和 2 3± 13pg/g、2 1± 10pg/g、19± 13pg/g ,ANP组较SO组有明显差异 (P <0 0 5 ) ,并随着时间而逐渐下降 ,而GLN组与SO组相比未见明显差异 (P >0 0 5 ) ,肠粘膜中CD+3 、CD+4 、CD+8和k+、λ+数量在ANP组中明显少于SO组 (P <0 0 5 ) ,GLN组中CD+3 、CD+4 。 Aim To observe the changes of intestinal immunity in acute necrotizing pancreatits(ANP) and effect of glutamine upon it Method Fifty four SD rats were randomly divided into 3 groups:①Sham operation(SO, n =18) ②ANP(ANP, n =18)③ANP+GlN(GLN, n =18) The model was induced by injection of 5% sodium taurocholate into biliopancreatic duct of rats The total parenteral nutrition(TPN) solution was given intraveneously, and the rats in GLN group were received 3% glutamine dipeptide (equal to 2% glutamine) Three groups were treated isonitrogenously The rats were sacrificed at 24,48,72h after operation respectively The percentage of intestinal bacteria coated by sIgA was measured by EIA and the concentraion of sIgA in gut was measured by RIA The number of CD + 3,CD + 4,CD + 8,T lymphocytes and k +、λ + B cells were counted by immunohistochemical assay Result At 24,48,72h,the percentage of intestinal bacteria coated by sIgA was decreased in ANP group but no change in GLN group,compared with SO group(50%,33%,33% vs 87%,100%,100% vs 100%,100%,100%) The concentration of sIgA in gut was obviously lower in ANP group than that in SO group and decreased as time on There was a significant decrease of CD + 3,CD + 4,CD + 8, T lymphocytes and k +,λ + plasma cells in lamina propria in ANP group The number of CD + 3,CD + 4,CD + 8 T lymphocyes and k +,λ + plasma cells in GLN group increased at 48,72h, but not at 24h Conclusions Glutamine supplement maintained the intestinal immune function in ANP rats, which benefited gut barrier function during the course of ANP
