摘要
目的 :通过研究 HP感染与胃癌及癌前病变中环氧合酶 - 2 (cox- 2 )表达的关系 ,以探讨 HP可能的致癌机制。方法 :经胃镜和病理诊断明确的病例共 12 0例 ,包括慢性浅表性胃炎 (CSG)、肠上皮化生、不典型增生、胃癌各 30例。以 ABC免疫组化法检测上述标本的 cox- 2 ,以改良 Giemsa法检测 HP。结果 :胃癌组肠型胃癌 HP阳性率为 83.3 % (2 0 / 2 4) ,弥漫型 6例中 HP阳性 1例。胃癌 HP阳性率为与 CSG比较差异非常显著 (P<0 .0 1) ,肠型胃癌 HP阳性率与胃癌阳性率比较有明显差异 (P<0 .0 5 )。各组 HP阳性的 cox- 2与其 HP阴性者作相应比较均有显著或非常显著性差异 (P<0 .0 5或 P<0 .0 1)。结论 :HP感染可使胃粘膜中 cox- 2过度表达 ,导致细胞凋亡受抑制 。
Objective:To explore the possible role of HP in the development of gastric cancer.The expression of cyclooxygenase 2(cox 2) in gastric cancer and precancerosis and their relation to HP infection were investigated.Methods:A total of 120 patients with chronic superficial gastritis(CSG, n =30),intestinal metaplasia(IM, n =30),atypical hyperplasia(AH, n =30)and gastric cancer(GCA, n =30) were studied.All subjects were diagnosed with endoscopy and histopathology.Expression of cox 2 were detected by immunohistochemistry(ABC method),and HP was examined by modified Giemsa.Rusults:Higher occurrences of HP infection were found in patients with gastric cancer than in patients with CSG( P <0.01).In patients with gastric cancer,HP positive rates were significantly related to intestinal type(20/24,vs diffused type 1/6, P <0.05).Expressin of cox 2 was intensive in HP positive patients when compared with HP negative patients( P <0.05 , P <0.01).These findings suggested that HP infection incur enhanced the expression of cox 2,suppressed cell apoptosis,and may be involved in the pathogenesis of gastric cancer.
