摘要
目的 研究马凡综合征 (MS)心血管病变及其并发症的MRI表现。材料与方法 符合MS诊断标准的患者 2 0例 ,行胸部MRI检查 ,采用自旋回波 (SE)序列及梯度回波 (GE)快速扫描序列。结果 升主动脉瘤样扩张 2 0例 ,其最宽处位于升主动脉根部(窦部 ) 18例 ( 90 % ) ,范围从根部至升主动脉远段 (弓前 )最常见 ( 10例 )。主动脉瓣环不同程度扩大 ( 10 0 % ) ,MRI电影准确显示主动脉瓣返流。左心室增大 2 0例 ( 10 0 % ) ,其中心腔扩大伴心肌肥厚 7例 ( 3 5 % ) ,单纯心腔扩大 12例 ( 60 % ) ,仅心肌肥厚 1例 ( 5 % )。主动脉夹层Ⅰ型 5例 ,其中 3例累及腹主动脉。肺淤血 6例 ,胸腔积液 7例 ,心包积液 4例。肺动脉高压 4例。左心房受压变扁 14例。结论 MS的MRI征象具有特征性 ,可以准确判断心血管病理、病理生理及心功能状况 ,能代替心血管造影检查制定手术方案。
Objective To study MRI features of the cardiovascular lesions and their complications in Marfan syndrome (MS).Materials and Methods Thoracic SE and GE fast scanning sequences were performed in 20 patients whose clinical data were up to MS diagnostic criterion.Results (1) All cases showed aneurysmal dilatation of the ascending aorta, in 18 patients the widest diameter was at the root (sinus) level. Most aneurysms extended from the root to the distal part of the ascending aorta (n=10). (2) Aortic valvular ring was enlarged in all cases. Cine MRI could well demonstrate aortic regurgitaion. (3) Enlarged left ventricle was seen in all patients, including enlarged chamber and thickened cardiac muscle in 7, only enlarged chamber in 12, and only thickened cardiac muscle in 1. (4) Aortic dissection of type I was found in 5 patients, of which the abdominal aorta was involved in 3. (5) Pulmonary congestion and/or edema (n=6), pleural effusion (n=7) and pericardial effusion (n=4) were found. (6) Pulmonary hypertension was complicated in 4 cases. (7) Due to the oppression from the aorta the left atrium was flattened in 14 cases.Conclusion MS carries some characteristics on MRI, by which the cardiovascular pathology, pathophysiology and cardiac function can be correctly judged, and optimal surgery plan be worked out.
