摘要
目的 探讨免疫低下时感染性肺炎是否存在肠源性感染。方法 将健康Wistar大鼠 48只随机分 4组 ,正常对照组、肺炎组、肺炎地米组、肺炎环磷组。后两组分别在建立肺炎模型前 10d分别连续腹腔注射地塞米松、环磷酰胺 ,每天 1次 ,第 11d同时建立绿脓杆菌肺炎模型。 2 4h后无菌操作下取出大鼠脏器、匀浆、细菌培养、菌落计数及细菌鉴定。结果 ①大鼠存活率 :对照组及肺炎组存活率 10 0 % ;肺炎地米组 40 % (6 / 15 ) ,肺炎环磷组 80 % (12 / 15 )。②脏器细菌培养阳性多见于肠系膜淋巴结、肝脏 ,细菌鉴定以革兰阴性杆菌为主。③细菌培养阳性率及菌落量 :肺炎地米组、肺炎环磷组的阳性率及菌落量明显高于对照组及肺炎组。结论 免疫低下时绿脓杆菌肺炎大鼠中存在肠源性感染。
Objective To investigate the existance of enterogenous infection in rats with infective pneumonia under immunosuppression.Methods 48 wistar rats were randomly divided into 4 groups:group of control,group of pyocyanic pneumonia(group PP),group of pneumonia treated with dexamethasone(group PD)and group of pneumonia treated with cyclophosphamide(group PC).Intrabdominal injection of dexamethasone and cyclophosphamide were given to the latter two groups once a day for 10 days successively before the pneumonia model was finished.On the 11th day,an animal model of pyocyanic pneumonia was established.After 24 hours,under asepsis,the rat' organs were separated and homogenated.At the same time,the bacteria cultur,total colony quantity and bacteria identification were detected.Results (1)Survival rate in both control group and group PP was 100%,in group PD 40%(6/15)and in group PC 80%(12/15).(2)In organ bacteria culture,the high positive bacteria rates were found in lymph nodes and liver.And gramnegative bacillns was the main pathogen in bacteria identification.(3)The posive bacteria culturerate and total colony quantity:positiverate and total colony quantity in both group PD and group PC were significantly higher than that in control group and group PP.Conclusion Enterogenous infection exists in rats with pyocyanic pneumonia under immunosuppression.
