摘要
目的 探讨天津地区汉族Graves病 (GD)白细胞减少易感性与HLA DRB1基因多态性的关联。方法 采用聚合酶链反应 序列特异性引物 (PCR SSP)方法检测 45例GD白细胞减少患者、5 0例GD白细胞正常患者和 90名正常对照的HLA DRB1等位基因频率。结果 (1)在不考虑白细胞变化的情况下 ,GD患者DRB1 0 8基因频率明显高于对照组 (P <0 .0 1,RR =2 .6 2 ) ,DRB1 0 7基因频率明显低于对照组 (P <0 .0 1,RR =0 .2 4)。 (2 )GD白细胞减少组DRB1 0 8(P <0 .0 1,RR =4.17)和DRB1 15 (P <0 .0 5 ,RR =1.6 9)基因频率较对照组显著增高 ,DRB1 0 7基因频率明显低于对照组 (P <0 .0 1,RR =0 .13)。 (3)GD白细胞减少组DRB1 0 8基因频率 (P <0 .0 1)和DRB1 15基因频率 (P <0 .0 5 )均明显高于白细胞正常组 ,DRB1 0 9(P <0 .0 5 )基因频率明显低于白细胞正常组。结论 天津地区汉族GD白细胞减少易感性与HLA DRB1 0 8,HLA DRB1 15基因频率增加有关 ;GD的保护性与HLA DRB1 0
Objective To investigate the relationship between HLA DRB1 gene polymorphism and susceptibility to leukopenia in patients with Graves′ disease (GD). Methods The HLA DRB1 alleles were typed by the polymerase chain reaction based sequence specific primer (PCR SSP) method in 45 GD patients with leukopenia, 50 GD patients without leukopenia and 90 normal controls. The allele frequencies in the leukopenic GD group were compared with those in the GD patients without leukopenia and control group. Results (1)Gene frequencies of HLA DRB1*08 (P<0.01, RR=2.62) are much higher and gene frequencies of HLA DRB1*07 (P<0.01, RR=0.24) are much lower in both GD groups than those in normal controls. (2)Gene frequencies of HLA DRB1*08 (P<0.01, RR=4.17), HLA DRB1*15 (P<0.05, RR=1.69) are much higher and HLA DRB1*07 (P<0.01, RR=0.13) are much lower in GD patients with leukopenia than those in normal controls. (3)The frequencies of HLA DRB1*08 (P<0.01) and HLA DRB1*15 (P<0.05) are significantly increased in GD patients with leukopenia as compared with those in GD patients without leukopenia, whereas HLA DRB1*09 (P<0.05) are significantly decreased. Conclusion HLA DRB1*08 and HLA DRB1*15 alleles are associated with the susceptibility to leukopenia in GD, while HLA DRB1*07 alleles are associated with the resistance of GD in Tianjin.
