脐血铅胎粪铅含量与新生儿神经行为发育的关系 被引量:9

摘要 目的 探讨在宫内低水平铅暴露状态下 ,新生儿脐血铅含量及胎粪铅含量与神经行为发育的关系。方法 随机选取武汉市青山区第一冶金建筑总公司职工医院妇产科 1999年 1~ 10月出生的足月新生儿 10 3例。以脐血铅和胎粪铅含量作为新生儿宫内铅暴露的指标 ,新生儿神经行为发育评分作为效应指标。采用描述性分析方法评价其相关关系。脐血铅以 0 483μmol/L为界、胎粪铅以 12 7 78mg/kg为界 ,分别将研究对象分为高铅组与低铅组。结果 脐血高铅组与低铅组仅在新生儿神经行为发育总评分 (135 2 5与 40 0 3 5 )、生物视听定向反应 (1310 5与 40 45 5 )得分上差异有显著性 ,但脐血铅含量与新生儿神经行为发育总评分、非生物听定向反应、非生物视定向反应、生物视听定向反应之间不存在等级相关关系 ;而胎粪高铅组与低铅组在新生儿神经行为发育总评分(16 36 0与 3 72 0 0 )、非生物听定向反应 (2 110 5与 3 2 45 5 )、非生物视定向反应 (1836 0与 3 5 2 0 0 )、生物视听定向反应 (182 8 5与 3 5 2 7 5 ) 4项评分上 ,两组差异有显著性或非常显著性。胎粪铅含量与新生儿神经行为发育总评分、非生物视定向反应、生物视听定向反应 3项神经评分有明确的等级负相关关系。 Objective To explore the relationship of lead levels in umbilical cord blood (CBPb) and meconium (MPb) with their neurobehavioral development in neonates with intrauterine exposure to lead at low level. Methods Levels of lead in umbilical cord blood and meconium were determined as indicators for intrauterine exposure to lead, and neonatal behavioral neurological assessment (NBNA) total score, and scores of non biological auditory orientation reaction(NBAOR), non biological visual orientation reaction(NBVOR) and biological visual and auditory orientation reaction(BVAOR)as effect indicators, were determined for 103 randomly selected neonates delivered at the Department of Obstetrics and Gynecology, Wuhan First Metallurgical Construction Company Hospital, Qingshan District during January to October 1999 A descriptive analysis was applied to evaluate their correlationship The neonates were divided into two groups with high and low exposure based on the cutoff values of CBPb of 0 483 μmol/L and MPb of 127 78 mg/kg. Results There was significant difference only in scores of NBNA(1 352 5 and 4 003 5)and BVAOR(1 310 5 and 4 045 5) between the groups of high and low lead levels in umbilical cord blood There was no rank correlation between lead level of umbilical cord blood and scores of NBNA, NBAOR, NBVOR and BVAOR And, there was significant difference in scores of NBNA (1 636 0 and 3 720 0), NBAOR(2 110 5 and 3 245 5), BVAOR(1 828 5 and 3 527 5) and NBVOR(1 836 0 and 3 520 0) between groups of high and low meconium lead levels The level of lead in meconium correlated reversely with scores of NBNA, NBVOR and BVAOR Conclusion Level of lead in meconium could be used as a more sensitive indicator for lead deposit in the fetal body during the period of pregnancy
