摘要
目的 观察经静脉注射造影剂后肿块区域灰阶变化 ,探讨原发性肝癌的血供特点。方法 2 3例原发性肝癌行间歇二次谐频声学造影 ,间歇成像由心电图R波触发 ,触发间隔为 1~ 2个心动周期。声学造影剂经左前臂浅静脉弹丸式注射。结果 注射造影剂后 15~ 18s肝组织显影 ,回声增强。肿块区域Ⅱ级增强 4例 ,Ⅲ级 19例。肿块内回声强弱不均 ,并可见多条粗细不等、迂曲杂乱走行的亮线状异常血管结构 ,呈“蜘蛛网”样形态。肿块区域早于周围肝组织 1~ 2个心动周期显影 ,晚于周围肝组织 15~ 2 0个心动周期 [平均 (17± 2 )个 ]消退 ,显影过程持续 10 0~ 12 0s[平均 (110± 5 )s]。结论 间歇二次谐频声学造影是检测组织血流的敏感方法 ,不仅能清楚显示癌瘤内较大的异常血管结构 ,还能观察其微循环血供状况 ,所获血流信息具有一定的特征性 。
Objective To investigate the characteristics of blood supply in hepatocellular carcinoma (HCC) by comparing gray scale changes of HCC before and after intravenous administration of contrast agent.Methods Twenty three cases of HCC were examined with intermittent second harmonic contrast imaging (ISHCI), the intermittent imaging were triggered by R wave of electrocardiogram (ECG) in the interval of 1 or 2 cardiac cycles. A “bolus like” injection was administered the contrast agent into left forearm veins. Results The echoes of parenchyma of liver were enhanced at 15~18 s after administration of contrast agents. Four cases of HCC nodules were enhanced to grade Ⅱ, 19 cases grade Ⅲ. The echoes of HCC nodules were heterogeneous, and many abnormal hyperplastic blood vessels were seen clearly to be bright lines, which were different in width, with their course tortuosity like a “spider's web”. Contrast agent appeared 1 or 2 cardiac cycles and disappeared 15~20 cardiac cycles later (mean 17±2), each total course lasting 100~120 s[mean (110±5)s] earlier in tumors than in their surrounding parenchyma. Conclusions ISHCI is sensitive in detecting blood flow of liver tissues, it can clearly show both the larger abnormal hyperplastic blood vessels in tumors and the blood supply of microcirculation. All the information about the blood supply in tumors display some characteristics, which may become a great help in the diagnosis or differentiation of HCC.
