精氨酸与谷氨酰胺对进展期胃癌患者免疫功能及肿瘤细胞增殖活性的影响 被引量:5

Effect of Arginine and Glutamine on the Immune Function and Tum or Cell Proliferating Activity in Patient with Advanced Gastric Cancer

摘要 目的 :研究联合应用精氨酸 (Arg)与谷氨酰胺 (Gln)对胃癌病人免疫功能和肿瘤细胞增殖动力学的作用。方法 :胃癌 5 8例 (男 4 2例 ,女 16例 )平均年龄 5 6.3± 11.4岁。术前每天静脉输注Arg 2 5 g、分 3次口服单体Gln粉剂 3 0g ,连续 7d。分别在用药前、后 (手术日晨 )检测细胞免疫指标CD3、CD4、CD8、CD4 CD8、NK细胞活性和体液免疫指标IgG、IgA、IgM、C3、C4 ,并用免疫组化法检测用药前胃镜活检标本和用药后手术切除标本的瘤增殖细胞核抗原 (PCNA)表达及细胞凋亡的变化。结果 :用药后CD3、CD4百分比明显提高 ,CD8百分比明显降低 ,CD4 CD8比值和NK细胞活性均明显提高 (P <0 .0 5 ) ;体液免疫指标无明显变化 (P >0 .0 5 ) ;瘤组织PCNA表达明显降低 ,而细胞凋亡明显增加 (P <0 .0 5 )。结论 :药理剂量精氨酸与谷氨酰胺联合应用能明显改善胃癌病人细胞免疫功能、抑制瘤组织PCNA表达并促进瘤细胞凋亡 ,但对体液免疫指标无显著影响。 Objective: To study the effect of a rginine and glutamine on the immune function and tumor cell proliferating activi ty in patients with advanced gastric cancer. Methods: 58 patient s with advanced gastric cancer, 42 males and 16 females, with the average age of 56.3±11.4, were studied. All patients recieved arginine 25g/d intravenously and glutamine 30g/d p.o 3 times per day for 7 days before operation. The ch anges of cellular immune parameters such as T lymphocyte subsets and natural kil ler (NK) cell activity, hormonal immune parameters were determined before and af ter the use of drug. Tumor proliferating cell nuclear antigen (PCNA) expression and tumor cell apoptosis were studied by immunohistochemical method. Res ults: There were significant improvements on CD3, CD4, CD4/CD8, and NK c ell activity after the therapy(P<0.05). The hormonal parameters did not change. PCNA expression of tumor was inhibited significantly (P<0.05). The t umor cell apoptosis was increased significantly (P<0.05). Conclusion : The combined application of arginine and glutamine in pharmaceutical d osage can improve cellular immunity, inhibit tumor cell PCNA expression and ac celerate tumor cell apoptosis in patients with advanced gastric cancer, but can not affect the hormonal immunity.
