摘要
目的 :探讨氯沙坦、卡托普利降压治疗后对原发性高血压 (EH )患者血管紧张素 (A )、醛固酮(AL D)及纤溶活性的影响。方法 :检测 、 期 EH患者 5 2例和正常对照者 2 6例的组织型纤溶酶原激活物 (t- PA )及其抑制物 (PAI- 1)的活性、血浆 A 、AL D水平。5 2例 EH患者中 2 6例接受氯沙坦治疗 ,2 6例用卡托普利治疗 ,疗程 4周。4周后复查上述指标。结果 :EH组 PAI- 1活性及血浆 A 、AL D水平均较正常对照组明显增高 (P <0 .0 5~ 0 .0 1) ,t- PA活性降低 (P <0 .0 0 1)。EH组治疗前 A 水平与 PAI- 1呈显著正相关 (P <0 .0 5 ) ,与 t- PA无相关 (P >0 .0 5 )。经氯沙坦、卡托普利治疗后显示 ,在明显降压的同时 ,t- PA活性增强 ,PAI- 1活性降低 ,与治疗前相比 ,差异有显著性意义 (P <0 .0 5~ 0 .0 1)。氯沙坦对 A 、AL D无明显影响 ,卡托普利使 A 、AL D水平明显下降 (P <0 .0 5 )。结论 :EH患者存在纤溶活性降低 ,且肾素 -血管紧张素系统激活与纤溶活性异常有密切关系。经上述两种药物治疗后 ,均能改善患者的纤溶活性异常。对循环中 A 、AL
Objective:To study the influenc e of losartan or Catopril on angiotensin Ⅱ (AⅡ), aldolase (ALD) and fibrinolyt ic function in essential hypertension (EH) patients.Method:Using spectrophotometric method and radioimmunoassa y, we tested the tissue type plasminogen activator (t-PA) and its inhibitor Ⅰ (PAI-Ⅰ) and AⅡ, ALD content in the plasma of 52 EH cases. In the meantime, 26 normal persons were taken as control group. In the beginning, 26 of 52 EH cases were treated with losartan and the other 26 cases were treated with catopril; and after 4 weeks treatment the above data were detected again in 52 EH cases. Result:The PAI-Ⅰ and AⅡ, ALD content in EH cases were obviously higher than those in control group (P< 0.05~ 0.01) but t -PA was lower (P< 0.001). AⅡ was interrelated to PAI-Ⅰ in EH patient s obviously before treatment (P< 0.05) but AⅡ was not interrelated to t -PA (P> 0.05).And t-PA content was increased with blood pressure (Bp) reduced after treatment of losartan or catopril. PAI-Ⅰ content was reduced o bviously after treatment (P< 0.05~ 0.01). Furthermore, losartan did not interfere AⅡ and ALD content but catopril could reduce them obviously (P < 0.05). Conclusion: The fibrinolytic system activty extent is low er in EH patients, and there is a closely relationship between activiation of re nin-angiotensin system (RAS) and abnormal fibrinolytic system activity. After t reated with losartan or catopril, the abnormal fibrinolytic system activity is i mproved. The influence on AⅡ and ALD in plasma after treated with losartan or c atopril is different.
