摘要
目的 :探讨兔单纯脑受压减压后脑肿大与相应脑血流参数改变的相关性。方法 :硬膜外水囊加压法建立兔急性颅内高压模型 ,分A、B、C 3组监测兔颅内压及基底动脉血流 ,于舒张期末流速消失时停止加压并分别维持颅内压于此水平 30min、6 0min、12 0min ,然后减压 ,并观察颅内压及脑血流至减压后 2 4h ,处死动物取全脑行病理检查。结果 :A、B组动物减压后 4h内脑血流恢复正常 ,B组动物 2 0 %发生脑水肿 ;C组减压 4h各指标均较减压30min时有显著改变 (P <0 .0 5 ) ,10 0 %发生脑肿大。结论 :单纯脑受压后 6 0min内减压 ,脑血流容易恢复 ,脑肿大发生率低 ;12 0min减压脑血流不恢复 ,脑肿大发生率极大。
Aim: To investigate the relation between brain edema and swelling after decompression of simple brain compression and simultaneous changes of the cerebral blood flow. Methods: Acute increased intracranial pressure model was developed by positive pressure irrigation by pushing fluid into an epidermal tiny bag beneath the skull.Thirty rabbits were divided into group A,B and C randomly,and intracranial pressure(ICP) in 3 gronps were maintained steadily for 30,60 and 120 min respectively while the V d=0,then decreased the pressure.Meanwhile the changes of the ICP and cerebral blood flow(CBF) of basilar artery (BA) were investigated.Then the animals' brains were taken for pathological examination. Results: After decompression,CBF reached normal in group A and B within 4 h,but ICP increased,the values of V d and resistance index (RI) changed 4 h later significantly compared with those of 30 min after decompression respectively,all the animals developed a brain bulk enlargement .Conclusion: It is indicated that in simple brain decompression,if V d (RI) does not reach normal 4 h after decompression and ever decreased (increased) gradually,brain bulk enlargement will be always unavoidable.
