摘要 目的 本实验通过研究神经母细胞瘤的增殖、凋亡的失衡 ,结合神经母细胞瘤的分化 ,以探讨增殖、凋亡的改变与神经母细胞瘤预后的关系。方法 分别取正常对照组、良性肿瘤组、恶性肿瘤组 3组组织制作石蜡切片 ,运用免疫组织化学法检测增殖细胞 ,计算增殖细胞占细胞百分比 ,求得增殖指数 (PI)。运用原位DNA末端标记法 ,检测凋亡细胞 ,计算凋亡细胞占细胞百分比 ,求得凋亡指数 (AI)。结合神经母细胞瘤的组织学分类 ,了解增殖指数、凋亡指数与神经母细胞瘤预后情况。结果 预后好的组织与预后差的组织 2组的PI相比P <0 .0 1。预后好的组织与预后差的组织两组的AI相比P <0 .0 1。Ⅲ期或Ⅳ期死亡与健在分别PI相比较P <0 0 1。Ⅲ期或Ⅳ期死亡与健在分别AI相比较P <0 0 1。结论 PCNA指数越高 ,神经母细胞瘤预后越差。凋亡细胞和凋亡小体越多 ,凋亡指数越高 ,神经母细胞瘤预后越好。联合检测神经母细胞瘤的增殖指数、凋亡指数结合凋亡小体可提高对神经母细胞瘤诊断及预后判断的准确性 。 Objective To search for the relation between the proliferation or the apoptosis and the 1 prognosis of human neuroblastoma.Methods PCNA was identified immunohistochemically and the percentage of the PCNA positive nuclei (PCNA index:PI) was calculated.Apoptosis was determined by in situ end labelling of fragmented DNA and the percentage of apoptosis -positive nuclei (apoptosis index:AI) was also calculated.The relation between proliferation or apoptosis and the prognosis of neuroblastoma of different histological classification and clinical staging was studied.The PI and AI of the control,the ganglioneuroma and neuroblastoma specimens were studied respectively.Results PI was getting higher and higher from the control group,subsequently to the ganglioneuroma group, and to the neuroblastoma group.Patients with higher PI had a worse survival rate in comparison with those with lower PI.PI of the unfavourable histology group to PI of the favourable histology group was P <0.01.High PI is significantly related with poor survival rate.In contrast,a high density of apoptosis indicated favourable outcome.Al got lower and lower from ganglioneuroma subsuquently to neuroblastoma.The survival rate of younger patients usually with higher AI,was much better than those of patients usually with lower AI.The same as:in AI of the unfavourable histology group to in AI of the favourable histology group ( P <0.01),in PI of the stage Ⅲ or the stage Ⅳ neuroblastoma survival group to in PI of the stage Ⅲ or the stage Ⅳ neuroblastoma death group,and in AI of the stage Ⅲ or the stage Ⅳ neuroblastoma survival group to in AI of the stage Ⅲ or the stage Ⅳ neuroblastoma death group ( P <0.01).Conclusions The higher the PI is,the poorer the neuroblastoma prognosis will be.The higher the AI is ,the better the neuroblastoma prognosis will be.It is important to evaluate the prognosis and to guide the treatment of neuroblastoma.PI and AI should be examined together.
