摘要
为探讨影响血浆凝血酶原时间 (PT)和活化部分凝血活酶时间 (APTT)检验误差的护理相关因素 ,观察血样本在不同采样顺序、不同保存温度和时间、不同束臂压力及不同采血剂量与抗凝剂比例等条件下 PT、APTT测定值。结果示 ,PT与 APTT测定值均受上述各因素的影响 1采血样本第 1管与第 2、 3管 PT测定结果比较 ,差异有极显著性意义 (P<0 .0 1)。 2样本在 4℃保存 4、 8h的 PT及 2 4h APTT测定结果与 1h比较 ,差异有显著性意义 (P<0 .0 1,P<0 .0 5 ) ;样本在 15℃保存 8、 2 4h的 APTT结果与 1h比较 ,差异有显著性意义 (P<0 .0 5 ) ;样本在 2 5℃保存 4、 8、 2 4h测定 APTT结果与 1h比较 ,差异有显著性意义 (P<0 .0 1,P<0 .0 5 ) ;样本在 35℃保存 4、 8、 2 4h两项值与 1h相比 ,差异有极显著性意义 (均 P<0 .0 1)。 3不同束臂压力抽血其 PT值比较 ,差异有极显著性意义 (P<0 .0 1)。 4采血量与抗凝剂比例 1∶ 5与 1∶ 9比较 ,PT、 APTT测定结果差异有极显著性意义 (均 P<0 .0 1)。提示在临床护理中 ,应把好样本采集、运送、保存等质量关 ,尽量减少人为因素对PT和 APTT值的检验误差 。
The nursing related factors influencing test error of prothrombin time (PT) and activated partial thromboplaslin time (APTT) were investigated. The PT and APTT were measured in the blood samples under the conditions as follows: different sampling order, different stored temperature and time, different bound pressure, different blood quantity and anticoagulant ratio. The results showed that the values of both PT and APTT were affected by the above factors. There was very significant difference in PT value between the first tube and second or third tubes ( P <0.01). There was very significant difference in PT at 4 h and 8 h and APTT at 24 h with the samples stored at 4℃ as compared with those at 1 h ( P <0.01, P <0 05). The APTT at 8 h and 24 h with the samples stored at 15℃ or 25℃ was significantly different from that at 1 h ( P <0.05, P <0.01). Both PT and APTT at 4, 8,24 h at 35℃ were very significantly different from those at 1 h (all P <0.01). PT was varied significantly with different arm bound pressure ( P <0.01). There was significant difference in PT and APTT between 1∶5 of sampling volume to anticoagulants and 1∶9 (all P <0.01). It was suggested that during clinical nursing care, it was essential to reduce the testing error of PT and APTT by artificial factors as far as possible.
