摘要
目的 :探讨带血管游离复合髂骨瓣修复一侧下颌骨缺损对颜面美学形态的影响。方法 :应用带血管蒂的游离复合髂骨瓣同期修复一侧下颌骨缺损 11例。通过远期随访观察下颌活动的形态美学特点。结果 :11例再造下颌骨全部成活 ,经远期随访观察 ,颌面基本对称者 7例 ,下颌角轻度向下外方突出者 2例 ,患侧比健侧丰满者 2例。所有患者牙合关系和语音功能良好 ,对手术效果表示满意。X线片检查无骨吸收和复发病例 ,下颌小范围活动时 ,无美学的形态异常 ,但当大张口时下颌均轻度偏向患侧 ,这与一侧下颌骨切除后升、降颌肌群的平衡和关节形态的改变有关。结论 :带血管游离复合髂骨瓣是修复下颌骨缺损的较好供骨 ,具有良好的美学效果。
Objective:To evaluate the effect of aesthetics on r ep airing defect of mandible with vascularized free iliac osseomyocutaneous flap. Methods:In order to observe long term effect of repairing mandibular d efect with revascularized free iliac bone graft, eleven patients were followed u p. Results:All flaps survived and lower jaw was almost symmetrical in seven cases. The mandible angles of two patients slightly protruded downwards an d outwards. Two patients had one protrusion on the mandible. All patients had a good occlusion and pronunciation, and satisfied with operating effect. There wer e no bone tissue absorption and recurrence of tumor on X ray film. The mo vement of reconstructed mandible was normal on aesthetic viewpoint, but when the mouth opened wide, the mandible deviated to repairing side slightly, which was related to balance of muscular groups elevating and dropping mandible and altera tion of joint morphogenesis. Conclusion:The vascularized free iliac os seomyocutaneous flap is appropriate for primary reconstructing of oromandibular defect after oncologic sugery operation, and has a good aesthetic effect.
