摘要
目的 探讨室缺并重度肺动脉高压的围术期处理。方法 连续 4例经心导管检查确诊的室缺并重度肺动脉高压患者在体外循环下行室缺补片修补术 ,术后应用有效的血管扩张药物 ,严密监测血流动力学 ,防治肺高压危象和肺部并发症。结果 所有 4例患者均安全度过围术期 ,临床痊愈出院 ,半年后随访 ,紫绀消失 ,自觉症状明显改善 ,肺动脉压降至 2 .46～ 5 .47k Pa,下降幅度达 5 0 %～ 75 % ,心功能均达 ～ 级。结论 只要严格掌握手术适应症 ,一部分室缺并重度肺动脉高压患者就可获得手术治疗机会并安全度过围术期 。
Objective\ To study perioperative management of patients of ventricular septal defect with severe pulmonary arterial hypertension.Methods Consecutive 4 patients of ventricular septal defect with severe pulmonary arterial hypertension underwent patch repairment operation under CPB.After operations,they were given effective vasodilator and monitoring of their haemodynamics.They were given medical treatment to prevent critical condition of pulmnary arterial hypertension and complications of vital organs.Results\ All 4 patients were safe during perioperative period.When they were follow uped six months later,they complained no cyanosis and improved physical condition.Pulmonary arterial pressure were lowered to 2 46～5 47kPa.Heart function reached Ⅰ～Ⅱ degree.Conclusion\ If surgeons strictly select patients and give effective perioperative management,some patients of ventricular septal defect with severe pulmonary arterial hypertension maybe surgically treated and recovered without complications.Pulmonry arterial pressure maybe lowered to almost normal range.
