摘要
目的 研究术前急性等容血液稀释对血液动力学和氧代谢的影响。方法 2 0例肝癌切除术病人 ,随机分为血液稀释组 (Ⅰ组 ,n =10 )和对照组 (Ⅱ组 ,n =10 )。Ⅰ组于全麻后采血 ,将血红蛋白稀释到 80~ 90 g/L ,同时输入两倍于采血量的晶胶体溶液。在稀释前后各时点监测血液动力学、氧代谢以及动脉血乳酸等参数。结果 Ⅰ组于血液稀释后 ,心脏指数 (CI)和每搏指数 (SI)维持稳定 ,氧输送量 (DO2 )降低 (P <0 0 1) ,但仍高于临界氧输送值。结论 在吸入高浓度氧的情况下 ,中度血液稀释无明显的心血管代偿反应 ,不引起氧输送不足。
Objective To evaluate the influences of acute hemodilution on hemodynamics and oxygenation.Methods Twenty adult patients with liver cancer undergoing hepatolobectomy were randomly allocated into two groups,hemodilution group(group Ⅰ, n= 10) and control group(group Ⅱ, n= 10).After induction,in group Ⅰ,phlebotomy was performed to reduce Hb to 80 90g/L by simutaneously infusing of colloid and crystal liquid.Hemodynamics,oxygen deliver,oxygen consumption and arterial blood lactate were measured before and after hemodilution.Results In group Ⅰ,CI and SI remained stable and DO 2 decreased significantly( P< 0 01) compared to those before hemodilution,which was still beyond the critical value of oxygen deliver.Conclusion With a high fraction of inspired oxygen,cardiovascular compensatory respone to moderate hemodilution may not take place,but DO 2 is still adequate.
