支气管扩张的螺旋CT动态扫描——空气潴留与支气管扩张、肺功能指数相互关系 被引量:2

摘要 目的 评价空气潴留、支气管扩张与肺功能指数相关性。方法 对 33例临床和HRCT检查确诊的支气管扩张的病人 ,使用螺旋CT分别进行了不同呼吸时相的连续容积数据模式扫描。对支气管扩张和空气潴留分别进行评分。 2 8例病人做了肺功能检查。结果 支气管扩张和空气潴留分别在 98/ 198和 112 / 198个肺叶中见到 ,两者平均积分高度相关 (r =0 .94;P <0 .0 0 1)。空气潴留积分与肺功能指数相关性如下 :FEV1、FVC、FEV1/FVC不相关 ,FIF2 5相关 (r =- 0 .5 1;P <0 .0 5 ) ,FIF50 (r=- 0 .77;P <0 .0 1)、RV(r =0 .75 ;P <0 .0 1)高度相关。结论 在支气管扩张病人中空气潴留与支气管扩张。 Purpose To evaluate the correlations of air trapping and bronchiectasis, pulmonary functional indexes. Methods 33 patients with bronchiectasis were diagnosed by clinical data and HRCT scan, and scanned using spiral CT at end inspiration and expiration in continuous volume scan mode. Expiration scans were scored at a lobar level for the extent of bronchiectasis and areas of decreased attenuation. 28 cases were performed pulmonary function tests. Results Bronchiectasis and areas of decreased attenuation were seen in 98/198 and 112/198 pulmonary lobars. The mean decreased attenuation score was high related to mean bronchiectasis scores ( r =0.94; P <0.001). The correlations of functional indexes and total decreased attenuation score was: Negative correlations were found in FIF 50 ( r =-0.77; P <0.01) and FIF 25 ( r =-0.51; P <0.05). Positive correlation was found in RV ( r =0.75; P <0.01). FEV 1, FVC and FEV 1/FVC did not correlate with total decreased attenuation score. Conclusion There were definite correlations of air trapping and bronchiectasis、small airways' functional indexes in patients with bronchiectasis.
