摘要
目的 探讨外伤性白内障现代囊外摘出联合人工晶状体植入术的手术时机及方式选择、手术技巧、人工晶状体植入、手术中特殊问题处理及效果等。方法 对２６３例外伤性白内障进行回顾性研究。结果２６３例外伤性白内障，术后随访１周矫正视力＞０．３为２２８例，术后近期并发症以角膜反应，瞳孔区纤维渗出膜形成，虹膜后粘连，前房出血等为多见。结论外伤性白内障由于病情的复杂，多样，其治疗比较特殊，目前尚无治疗外伤性白内障的特效药物，对于晶状体浑浊的患者，手术仍是首选，人工晶状体植入术是治疗外伤性白内障最理想的方法，增视效果好，应该尽早手术以防止弱视，并促进双眼单视和立体视觉的恢复。
Objective To explore the operative occasion and the surgical methods of selection, operative skill, intraocular lens implantation, special problem treatment during operation and surgical effects, etc. with ECCE intraocular lens implantation for the treatment of traumatic cataract. Methods Retrospective study of 263 traumatic cataract cases. Result Corrected visual acuity > 0. 3 in 228 of 263 cases at 1 week postoperative follow-up. There were postoperative complications such as corneal response, formation of flberous membrane in the pupil area, posterior synechia of the iris , hyphema etc. Conclusion The treatment of traumatic cataract is quite special as the patients' condition are diversified . There is no medicine founded to treat traumatic cataract until now. Surgery is still the first selection for the cases of lenticular opacity. And intraocular lens implantation is the most optimized method to treat traumatic cataract with good effect on increasing vision acuity. Surgery should be done as early as possible so as to prevent amblyopia, improve binocular single vision and stereoscopic vision.
