摘要
目的 :探讨股四头肌成形术并发症发生的原因及其处理方法。方法 :对 2 0 6例股四头肌成形术进行回顾性分析 ,其中 18例分别于术中或 /和术后产生并发症 ,包括伸膝装置损伤、皮肤损害、伸膝无力、再次粘连和罕见的股骨后髁骨折 ,所有病例均予相应处理。结果 :平均随访 4.7年 ,膝关节活动度最大 12 0°～ 0°～ 5° ,最小 5 0°～ 3° ,平均 98.6°～ 0°～ 1.1°。结论 :发生并发症的原因为适应证选择欠妥、切口失当、操作粗暴、错误延长股直肌腱、粘连松解不彻底、术后引流不畅和功能锻炼失误 ;推荐应用股外侧切口、股直肌起点剥离。
Objective: To investigate the etiology and therapy for complications in quadricepsplasty.Method:Retrospective analysis were done for 206 cases of quadricepsplasty,among which,complications occurred in 18 cases intraoperatively or postoperatively,including injuries caused by knee extending devices,skin damage,weakness in knee extension readhesion and rare posterior condylar fractures.All the cases were treated correspondingly.Result:The average follow up was 4.7 years,the maximum motion range of the knee joint was 120°～0°～5°,and the minimum 50°～3°,(averaged 98.6°～0°～1.1°).Conclusion:The etiology of complications were improper indication,incorrect incision,rough operative maneuve,wrong lengthening of femoral rectus tendon,incomplete release of adhesion,inadequate drainage and rough functional exercise.It is recommended that lateral incision,rectus stripping from the initial point,smooth closing of articular capsule and early active or passive functional exercise.
