摘要
目的 观察乙型肝炎病毒通过产妇传播在胎儿肾脏组织中表达的情况。方法 采集我院 4 0例乙型肝炎产妇娩下的死胎 ,常规尸检 ,取肾脏组织 ,SP法检测HBsAg ;回访婴母产前静脉血HBV的检测结果 ;计数资料行 χ2 检验。结果 HBsAg阳性颗粒在死胎的肾实质细胞 (近、远曲小管上皮细胞和肾血管球毛细血管内皮细胞 ,球内、外系膜细胞 ,球旁细胞 ,足细胞 )浆、肾脏血管中呈点、灶状分布 ,细胞核不着色。HBV呈单项阳性、小三阳、大三阳的婴母分娩的死胎 ,肾脏组织中HBsAg阳性率比较 ,差异不显著 (P >0 0 5 )。
Objective To observe the expression of HBsAg in the kidney tissue of the dead fetus infected by HBV by maternal-fetal transmission.Methods 40 cases dead fetus laboured by the puerperal with hepatitis virus B were made ordinary autopsy for kidney tissues to detected HBsAg with immunocytochemistry(SP).Investigated the detection on HBV in the mother veins before delivery.Results HBsAg was mainly in the kidney cells(proximal or distal convoluted tubule epithelial cell,glomerulus capillary endothelium cell,intraglomerular or outraglomerular mesangial cell,juxtaglomerular cell,podocyte)and vessels.HBsAg was not in the kidney cells nuclei.HBsAg were single or in clusters form distributed over in the kidney vessel and kidney cells plasma.For the dead fetus laboured by the puerperal with hepatitis virus B,as only one of HBVM+,(HBsAg+HBeAb+HBcAb) and(HBsAg+HBeAg+HBcAb)in the mother veins before delivery,it was not statistically significant (P>0 05)for the comparison among the rate of the HBsAg positive expression in the kidney tissues.Conclusion It was not correlated with the HBVM in the pregnant woman veins for the rate of HBsAg in the kidney.
