硬膜外麻醉下体表温度变化及与核心温度变化的关系 被引量:6

The changes of body surface temperatures and their association with the changes of core temperatures in patients with epidural anesthesia

摘要 目的 观察硬膜外麻醉下阻滞和非阻滞区域体表温度变化及与核心体温变化的关系。方法 下腹部、会阴部和下肢单侧小手术病人 2 2例 ,随机在硬膜外腔注入注射用水 (对照组 )或 2 %利多卡因 (实验组 ) ,观察两组患者注液前、注液后 5、10、15、2 0、2 5、3 0、3 5、40和 45 min的腋温、足背和指尖温度及前臂 -指尖温度差的变化 ,以腋温作为核心温度 ,足背或指尖温度作为阻滞或非阻滞区域体表温度。结果 对照组腋温轻微升高 ,足背温度轻微降低 ,指尖温度明显升高 ,臂 -指温度差逐渐降低 ,由 >0℃变为 <0℃ ;实验组腋温明显降低 ,其变化与对照组比较有显著性差异 ,足背温度显著升高 ,指尖温度逐渐降低 ,臂 -指温度差明显升高。结论 硬膜外麻醉下核心温度降低与阻滞区域体表温度升高有关 ,体温调节使非阻滞区域外周血管收缩 。 Objective To observe the changes of body surface temperatures in block and non block area and their association with the changes of core temperatures in patients with epidural anesthesia.Methods The 22 patients scheduled for minor operation in lower abdomen,perineum and unilateral lower extremity were randomly assigned to epidurally injectable water (control group) or 2 percent lidocaine (experiment group) group.The temperatures in axilla,dorsal foot,fingertips and forearm fingertips temperature gradient were observed at the time intervals before epidural injection,5,10,15,20,25,30,35,40 and 45 minutes after epidural injection.The axillary temperatures were taken as core temperature,and the temperatures in dorsal foot or fingertip as surface temperatures in block or non block area.Results In control group,axillary temperature increased slightly,temperature in dorsal foot decreased slightly,fingertip temperatures increased significantly,and forearm fingertip temperature gradients gradully decreased from more than 0℃ to less than 0℃.In the experiment group,axillary temperature decreased markedly,there was significantly difference in its changes compared with those of control group,temperatures in dorsal foot increased significantly,fingertip temperatures decreased gradually forearm fingertip temperature gradients increased evidently.Conclusions The decrease of core temperatures under epidural anesthesia is correlated with the increase of body surface temperatures in block area,peripheral vasoconstriction and decrease of surface temperatures in non block area occure under thermoregaletion.
