颈椎病突出椎间盘组织炎性反应特性的研究 被引量:51

摘要 目的 :探讨颈椎病发生中突出颈椎间盘组织的炎症反应特性。方法 :取 31例脊髓型颈椎病患者的 35个突出的颈椎间盘标本和 3个成年人的 7个正常颈椎间盘标本 ,将每个标本分为 2份 ,1份作组织学检查 ,观察有无炎细胞浸润 ,1份用生物化学方法测定其中IL 1α、IL 6和TNF α的含量。结果 :35个突出颈椎间盘中 ,18个(5 1 4 % )在边缘区域有大量炎细胞浸润 ,其余 17个 (48 6 % )未见炎细胞浸润 ,对照组未见炎细胞浸润 ;35个突出颈椎间盘组织中IL 1α、IL 6和TNF α含量明显高于正常椎间盘。结论 :颈椎病患者突出椎间盘组织具有炎症反应特性 ,炎症反应可能在颈椎间盘退变和颈椎病的发生发展中起重要作用。 Objective:To study the inflammatory characteristic of herniated cervical intervertebral discs from patients with cervical spondylotic myelopathy,and the roles of the inflammation in the cervical disc degeneration and cervical spondylosis.Method:35 herniated cervical discs were obtained from 31 patients with cervical spondylotic myeloyathy during the anterior cervical surgery.7 normal,nondegenerated cervical discs(control group) were obtained from three fresh cadavers.All discs were divided into two samples, one of which was used as histologic examination,and the other was used to detect contents of IL 1α,IL 6 and TNF α biochemically.Result:In 35 herniated cervical discs,18(51 4%) were shown abundant inflammatory cell infiltration in margin of herniated discs;17(48 6%) no inflammatory cell infiltration;normal discs also no any inflammatory cell infiltration.Biochemical assay indicated that the contents of cytokine IL 1α,IL 6 and TNF α were obviously higher than that of normal discs.Conclusion:Herniated cervical disc from the cervical spondylotic myelopathy may be inflammatory,and inflammation may play an important role in cervical disc degeneration and in pathogenesis of cervical spondylosis.
