摘要
本文报道了应用三维有限元方法对腰椎间盘应力分布的分析结果。正常椎间盘纤维环在人体直立位、前屈位和后伸位时应力明显集中于后侧与后外侧,平均应力水平以后伸位时最高(P<0.01)。当椎间盘退变后,纤维环的平均应力水平比正常时显著降低(P<0.01)。这些结果具有一定的临床意义。作者认为,腰椎间盘突出症的发生与其局部应力异常增高有关,而椎间盘的退变又将增大腰椎其余部分的承载。作者还就腰椎屈伸活动对于椎间盘的作用进行了讨论。
The stress distribution of lumbarintervertebral disc is studied with athree-dimensional finite elementmodel. It is shown that the annulusfibrosis of the normal disc has a highlevel of stress in the posterior andpostero-lateral parts in upright, fle-xion and extension position, and theaverage stress in extension is thelargest (p<0.01), and that the annu-lus/fibrosis in the degenerative disehas a significantly decreased stresslevel (p<0.01). These results areof clinical significance. The lumbardise herniation is associated with theabnormal increase of local stress,while the degenerative change of thedisc produces overloading on theother parts of the lumbar spine. Inaddition, the effect of flexion-extension motion of the lumbar spineon the dise is discussed.
