人工血管移植修复肢体血管缺损及临床观察 被引量:29

Clinical application and observation of artificial blood vessel graft for vascular injuries in limbs

摘要 目的 总结应用人工血管移植修复肢体血管闭塞及缺损的临床效果。 方法  1992年 6月~ 2 0 0 0年 12月 ,采用Gore Tex膨胀泡沫聚四氟乙烯 (ePTEE)人工血管移植修复肢体动脉及静脉缺损 ,动 静脉造瘘及复合皮瓣移植、神经移植共 65例。 结果 创伤性下肢肢体人工血管移植有 1例行截肢术 ,桡动脉重建术中有 3例闭塞 ,1例血管旁路重建术后 3d并发急性心肌梗死而死亡 ,其余血管移植后肢体液血循环良好。术后行彩色多普勒检查、螺旋CT及DSA造影显示血管通畅。动 静脉造瘘术2 8例全部成功 ,全部患者在临床上定期进行血液透析。随访 2~ 5年 ,血管通畅率为 71%。 结论 人工血管移植修复肢体血管缺损、血管旁路术及动 静脉造瘘术是一种有效的手术方式。既能解决自体血管来源困难的难题 ,又能保证临床应用的效果。 Objective The purpose is to report the clinical results and observations of repairing vascular injuries and occlusion in limbs Methods From June 1992 to October 2000,65 cases with renal failure and vascular injuries or occlusion in limbs were received by polytetrafluoroethylene (ePTFE) artificial blood vessel grafting for arteriovenous fistulization and vascular defection or bypass reconstruction Results Five cases failed with limb amputation or myocardial infarction The follow up ranged from 2 to 5 years The primary patency rate of 71%was observed The results of the blood flow in graft was satisfactory and good condition of blood circulation at distal part of the extremity,and the artificial blood vessels could be punctured repeatedly and performed hemodialysis periodically Conclusion The artificial blood vessel grafting for vascular injuries and bypass in limbs and arteriovenous fistulization is a safe and convenient technique in clinical practice It can be a alternative method to solve the difficult source of autovessels in the body
