心脏的工程性研究

摘要 目的为临床医务工作者拓宽专业面、提高学术水平,为医学工程科研工作者和用工程方法治疗心脏病的研究者提供理论支持。方法根据心脏的解剖认知与生理数据,用工程力学和生物力学的基础理论进行定性与定量分析。结果澄清血液循环系统的概念,它不是体循环与肺循环两个循环的并联而是两个路径的串联:心脏是串联着的两个二级压缩泵;左右房室瓣膜数量的差别、四个腔室协调性要求的差别都是符合工程物理原理的"最佳设计";用工程方法治疗心脏的某些疾病是医学方法不可取代的;提出"传导搭桥"的新概念;阐明为什么完全植入式的人工心脏工程上最终的瓶颈将是能量支持系统而非血泵。结论离开生物力学知识无法全面解释心脏的奥妙结构与功能,不利于开发新的工程治疗器械与方法。人工心脏应前瞻性地重视能量支持系统的研究。 [Objective]To broaden the professional field for clinical medical workers and improve their academic level, and to provide theoretical support for medical researchers who treat cardiac disease with engineering method.[Methods]Based on anatomical and physiological data of hearts,the basic theory of engineering mechanics and biomechanics were applied for both qualitative and quantitative analysis.[Results]This paper theoretically amended the connotation of cardiovascular system,which means the pulmonary and systemic circulations are indeed a series of two routes rather than a series of two circuits;it is pointed out that the heart is composed of two second-stage compression pumps (left and right)which are in series connection;it is pointed out that the distinction between the left and right atrioventricular valves and the coordination of four cardiac chambers are the most optimized design according to the engineering physics principle;the use of engineering methods in the treatment of some cardiac diseases can not be substituted by medical methods so far;a new concept of "wired conduction bypass"was proposed;it clarified the reason why the final engineering bottleneck of an integrally implantable total artificial heart would be its energy support system instead of blood pumps.[Conclusion]Without the knowledge ofbiomechanics,it is impossible to completely explain the ingenious structure and function of hearts,which is not in favour of the development of new cardiac diseases treatment with engineering methods and equipment.Research on integrally implantable total artificial heart should pay more attention to the prospective study of energy support systems.
