摘要
目的分析眼内期视网膜母细胞瘤(retinoblastoma,RB)患儿血清神经元特异性烯醇化酶(neuron-specificenolase,NSE)、乳酸脱氢酶(1actate dehydrogenase,LDH)水平变化及临床意义。方法回顾性分析38例初诊为RB眼内期患儿的临床资料,根据其预后分为中枢神经系统(central nervous system,CNS)转移组10例、骨髓转移组8例和无转移组10例,比较3组化疗前、化疗第3、6个周期患儿血清NSE、LDH水平。结果3组化疗前血清NSE、LDH水平比较差异均无统计学意义(P>0.05);化疗前、化疗第3、6个周期CNS转移组血清NSE[(25±15)、(52±39)、(63±40)μg/L]水平及骨髓转移组血清LDH[(138±27)、(331±65)、(485±102)u/L]水平均依次升高(P<O.05),其余各组化疗前、后血清NSE、LDH水平比较差异均无统计学意义(P>O.05)。结论血清NSE水平可作为眼内期RB患儿早期CNS转移的实验室指标;血清LDH水平与眼内期RB患儿早期骨髓转移有关,持续增高意义较大。
Objective To analyze the changes of neuron-specific enolase (NSE)and lactate dehydrogenase (LDH)in the serum of children with retinoblastoma (RB)during intraocular period and clinical significances.Methods Thirty-eight children with initially diagnosed RB during intraocular period were retrospectively analyzed and were divided into central nervous system (CNS)metastasis group (n=10),bone marrow metastasis group (n=8)and no-metastasis group (n= 10)according to the prognosis.The NSE and LDH levels were detected and compared before chemotherapy as well as in the 3rd and 6th cycles of chemotherapy among three groups.Results No significant differences were found in the levels of NSE and LDH among three groups before chemotherapy (P>0.05).The levels of NSE ((25±15),(52±39),(63±40)μg/L)and LDH ((138±27),(331±65),(485±102)u/L)increased gradually in turns before chemotherapy as well as in the 3rd and 6th cycles of chemotherapy in CNS metastasis group (P<0.05),and showed no significant differences in the other two groups (P>0.05).Conclusion Serum NSE level can be used as a marker for early CNS metastasis in RB children during intraocular period.The serum LDH level is correlated with early bone marrow metastasis in RB children during intraocular period,and its steady increase is of a great significance.
