头穴针刺配合带针康复训练治疗儿童精神发育迟缓的临床研究 被引量:16

摘要 目的:观察针刺头部穴位配合带针康复训练与单纯康复训练在精神发育迟缓患儿智力商数及适应行为能力方面的临床疗效差异。方法:将56例精神发育迟缓患儿随机分成治疗组和对照组,每组各28例。两组患儿采用同样的语言行为、感觉统合等康复训练。治疗组使用头穴针刺,并进行带针训练,而对照组采用单纯的干预训练。在治疗前以及3个疗程后运用中国-韦氏儿童智力量表(C-WISC)和婴儿-初中生社会生活能力检查量表(S-M)进行言语智商(VIQ)、操作智商(PIQ)、全量表智商(FIQ)及适应行为能力评估并评价临床疗效。结果:智力方面,两组病例在治疗后VIQ、PIQ、FIQ均显著高于治疗前(P <0. 05);治疗组的FIQ、PIQ较对照组提高显著(P <0. 05);治疗组在知识、词汇、领悟、背数、图片排列、积木图及图形拼凑方面提升明显(P <0. 05),对照组在知识、积木图及图形拼凑方面提升明显(P <0. 05),治疗组在词汇及图片排列两方面较对照组提高显著(P <0. 05)。适应行为能力方面,两组病例在治疗后总评分均显著高于治疗前(P <0. 05),且治疗组较对照组提高显著(P <0. 05);其中治疗组在6个分项目中均提升明显(P <0. 05),对照组在独立生活、运动、参加集体活动项目上提升明显(P <0. 05);治疗组在运动、作业、交往及自我管理4个项目较对照组提高显著(P <0. 05)。结论:头穴针刺配合带针康复训练对儿童精神发育迟缓疗效明显,优于单纯的康复训练。 Objective:To observe the clinical difference between scalp acupuncture combined with rehabilita-tion training and single rehabilitation training in intelligence quotient and adaptive behavior ability in mentallyretarded children. Methods:56 children with mental retardation were randomly divided into the treatment group(n =28) and the control group(n =28). The two groups were treated by the same rehabilitation training of lan-guage behavior and sensory integration; on which basis,the treatment group was also treated with scalp acu-puncture,with needles retaining during the rehabilitation training. Chinese Wechsler Intelligence Scale(C -WISC) and social life ability scale(S - M) were used to assess VIQ,PIQ,and FIQ,as well as adaptationability and clinical effect in the two groups before and after three courses of the treatment. Results:In terms ofintelligence,the scores of VIQ,PIQ and FIQ were significantly increased in the both groups after the treatment(P <0. 05); the treament group was superior to the control group in terms of the improvements of FIQ and PIQ(P <0. 05). The treatment group improved more significantly in the knowledge,vocabulary,comprehension,number remembering,picture arrangement,piecing the block diagram and graphics together(P < 0. 05); thecontrol group improved more significantly in the knowledge,as well as in piecing the block diagram andgraphics together(P <0. 05). In terms of adaptive behavior after the treatment,the total score was significantly higher after the treatment than that before the treatment in the two groups(P <0. 05); of which the treatmentgroup improved more significantly than the control group(P < 0. 05); the treatment group improved in all sixsub - projects(P <0. 05),whereas the control group was only improved in the independent life,exercise andparticipation in group activities(P < 0. 05); the treatment group showed significant improvement in exercise,operation,communication and self - management than the control group did(P < 0. 05). Conclusion:Scalpacupuncture combined with rehabilitation training with retaining needls is effective for mental retardation ofchildren,which is superior to the single rehabilitation training.
