固本舒筋汤配合牵引、穴位电透入治疗神经根型颈椎病的疗效及对电生理指标和血浆NO、ET水平的影响 被引量:11

Effects of Using Guben Shujin Decoction Combined with Traction and Acupoint Iontophoresis in the Treatment of Nerve Root Type Cervical Spondylosis and the Influence on Electrophysiological Indexes and Plasma NO and ET Levels

摘要 目的:观察固本舒筋汤配合牵引、穴位电透入治疗神经根型颈椎病效果及对电生理指标和血浆一氧化氮(NO)、内皮素(ET)水平的影响。方法:选取我院2015年4月~2017年4月116例神经根型颈椎病患者为研究对象,按随机数表法分为观察组与对照组,每组58例。对照组给予牵引与穴位电透入治疗,观察组在对照组基础上给予固本舒筋汤,疗程均为6周。检测患者血管内皮功能与电生理指标,比较两组疼痛评分,评估中医证候疗效。结果:观察组治疗6周后NO为(65. 32±11. 75)μmol/L显著高于对照组的(57. 49±10. 36)μmol/L (P<0. 05),ET为(72. 67±13. 08) ng/L显著低于对照组的(80. 34±14. 46) ng/L (P<0. 05)。观察组治疗6周后腕部与肘部的正中神经、尺神经F传导速度分别为(53. 79±9. 69) m/s、(49. 32±8. 88) m/s、(55. 61±10. 02) m/s、(56. 38±10. 15) m/s显著高于对照组的(50. 23±9. 03) m/s、(46. 03±8. 34) m/s、(51. 92±9. 35) m/s、(52. 54±9. 46) m/s (P<0. 05)。观察组治疗6周后现在疼痛状况(PPI)、疼痛分级指数(PRI)与疼痛视觉模拟评分(VAS)显著低于对照组(P<0. 05)。结论:固本舒筋汤可以调节血浆NO、ET,改善血管内皮功能,修复受损神经根,配合牵引、穴位电透入治疗神经根型颈椎病,可以显著减轻患者疼痛症状。 Objective: To study the effect of using Guben Shujin Decoction combined with traction and acupoint iontophoresis in the treatment of nerve root type cervical spondylosis and the influence on electrophysiological indexes and plasma nitric oxide( NO) and endothelin( ET) levels. Methods: 116 patients with nerve root type cervical spondylosis who were admitted to the hospital from April 2015 to April 2017 were selected as the subjects. They were randomly divided into observation group and controlled group,58 cases in each group. The controlled group was treated with traction and acupoint iontophoresis,and on the basis,the observation group was additionally treated with Guben Shujin Decoction. The course of treatment was 6 weeks. The vascular endothelial function and electrophysiological indexes were detected. Pain scores were compared between the two groups,and the curative effect of TCM syndrome was evaluated. Results: The NO level in the observation group after 6 weeks of treatment was significantly higher than that in the controlled group [( 65. 32± 11. 75) μmol/L vs( 57. 49± 10. 36) μmol/L]( P < 0. 05),while the ET level was significantly lower than that in the controlled group [( 72. 67±13. 08) ng/L vs( 80. 34±14. 46) ng/L]( P < 0. 05). The F conduction velocities of wrist and elbow median nerve and ulnar nerve in the observation group after 6 weeks of treatment [( 53. 79±9. 69) m/s,( 49. 32±8. 88) m/s,( 55. 61±10. 02),m/s,( 56. 38±10. 15) m/s] were significantly higher than those in the controlled group [( 50. 23±9. 03) m/s,( 46. 03±8. 34) m/s,( 51. 92±9. 35) m/s,( 52. 54±9. 46) m/s]( P < 0. 05). The present pain index( PPI),pain rating index( PRI) and pain visual analogue scale( VAS)score were significantly lower in the observation group than in the controlled group after 6 weeks of treatment( P < 0. 05). Conclusion: Guben Shujin Decoction can regulate plasma NO and ET levels,improve the vascular endothelial function and repair damaged nerve roots. The combined use with traction and acupoint iontophoresis for treating nerve root type cervical spondylosis can significantly relieve pain symptoms in patients.
