摘要
目的观察动作观察疗法(AOT)对脑卒中后言语失用(AOS)的疗效。方法将38例AOS患者按随机数字表法分为AOT组(19例)和对照组(19例)。对照组给予每日2次语言康复训练,每次30 min,每周5 d,疗程4周。AOT组在对照组基础上,于语言功能训练前对患者进行AOT,每次20 min,每天1次。采用中国康复研究中心版的AOS评定方法以及汉语失语症成套测试(ABC)于治疗前后评价患者的语言功能;采用波士顿诊断性失语症检查(BDAE)评价治疗前后患者失语严重程度,并计算治疗显效率及有效率。结果对照组与AOT组患者治疗前AOS评分、ABC测试评分差异无统计学意义(均P> 0. 05)。与治疗前比较,两组患者治疗后AOS评分及ABC测试评分均显著增加(均P <0. 05)。AOT组治疗后AOS评分及ABC测试评分均显著高于对照组(均P <0. 05)。AOT组有效率显著高于对照组(χ2=6. 269,P=0. 012)。结论 AOT可显著改善脑卒中后AOS患者的语言功能。
Objective To evaluate the efficacy of action observation therapy (AOT) on apraxia of speech (AOS) in patients after strok.Methods Thirty-eight patients diagnosed as apraxia of speech after stroke were randomly divided into AOT group (n =19) and control group (n=19).Both group received twice conventional language therapy every day,30 min each time,5 d a week for 4 weeks.The patients in AOT group additionally received AOT before the first language therapy,20 min each time,5 times per week for 4 weeks.The speech function andaphasia severity of the two groups were assessed by speech apraxia assessment method of China Rehabilitation Research Center Edition,Aphasia Battery of Chinese (ABC ) and the Boston Diagnostic Aphasia Examination (BDAE) before and after treatment.Results There was no significant difference in scores of AOS and ABC between two groups(all P >0.05).The scores of AOS and ABC were significantly increased after treatment in both groups (all P <0.05).The scores of AOS and ABC in AOT group were significantly increased compared with the control group after the treatment (all P <0.05).The response rate in the test group was significantly higher than that in the control group (x^2=6.269,P =0.012).Conclusion Action observation therapy can significantly improve language function of patients with AOS after stroke.
