骨折患者血浆纤维蛋白原和碱性磷酸酶水平的变化及临床意义 被引量:11

Changes and clinical significance of plasma fibrinogen and alkaline phosphatase in patients with fracture

摘要 目的观察骨折患者血浆纤维蛋白原(FIB)和碱性磷酸酶(ALP)水平变化特点,探讨其与创伤程度、下肢深静脉血栓形成(DVT)的关系。方法选择2015年5月至2017年6月间西安市未央区中医医院骨科诊治的100例骨折患者为观察组,依据损伤严重程度(ISS)评分将患者分为轻度创伤组(42例)、中度创伤组(35例)和重度创伤组(23例),根据术后是否发生DVT将患者分为DVT组(12例)和NDVT组(88例)。选择100例门诊体检健康者为对照组,检测并比较各组受检者血清FIB、ALP水平的差异;分析FIB、ALP与创伤程度、DVT形成的关系。结果观察组患者的血清FIB和ALP水平分别为(4.51±1.62) g/L、(172.52±38.73) U/L,明显高于对照组的(2.62±0.51) g/L和(125.85±26.95) U/L,差异均有统计学意义(P<0.05);观察组患者的血清FIB、ALP水平均随创伤程度的加重而增高,差异有统计学意义(P<0.05);观察组患者血清FIB、ALP均与ISS评分呈正相关(r=0.302、0.519,P<0.05);DVT组患者的血清FIB、ALP水平分别为(5.62±1.78) g/L、(235.52±41.53) U/L,明显高于NDVT组的(2.57±0.49) g/L、(165.85±32.98) U/L,差异均有统计学意义(P<0.05)。结论骨折患者血浆FIB、ALP水平较正常人增高,检测血浆FIB、ALP水平可辅助评估骨折患者创伤程度,预防DVT。 Objective To observe the changes of plasma fibrinogen (FIB) and alkaline phosphatase (ALP) levels in patients with fracture, and to explore the relationship between FIB and traumatic degree, deep vein thrombosis (DVT). Methods From May 2015 to June 2017, 100 patients with fracture in Xi'an Weiyang District Chinese Medicine Hospital were selected as observation group, which were divided into mild trauma group (42 cases), moderate trauma group (35 cases), and severe trauma group (23 cases) according to the severity of injury (ISS). The patients were divided into DVT group (12 cases, with deep vein thrombosis) and NDVT group (88 cases, without deep vein thrombosis). The serum levels of FIB and ALP were measured and compared in 100 healthy outpatients. The relationship between FIB, ALP and the degree of trauma, DVT formation was analyzed. Results The levels of serum FIB and ALP in observation group were (4.51±1.62) g/L, (172.52±38.73) U/L, significantly higher than (2.62±0.51) g/L and (125.85±26.95) U/L in control group (P<0.05). The levels of serum FIB and ALP in observation group increased with the severity of trauma (P< 0.05). The serum FIB and ALP levels in the observation group were positively correlated with ISS scores (r=0.302, 0.519, P<0.05). The levels of serum FIB and ALP in DVT group were (5.62±1.78) g/L, (235.52±41.53) U/L, significantly higher than (2.57±0.49) g/L, (165.85±32.98) U/L in NDVT group, respectively (P<0.05). Conclusion The plasma levels of FIB and ALP in patients with fracture are higher than those in normal people. Detection of plasma levels of FIB and ALP can assist in evaluating the degree of trauma and preventing DVT in patients with fracture.
