摘要
目的工作满意度直接影响医务人员工作积极性。本研究旨在调查县级公立医院改革期间医务人员工作满意度中的职业风险现状,探究其影响因素,为有关部门制定调动医务人员积极性的政策提供科学依据。方法采取整群抽样的方法,选取2016-09-05—2017-03-08贵州省7家县级公立医院的2 532名医务人员作为研究对象,对其进行问卷调查,采用单因素分析和多元逐步回归分析其职业风险的影响因素。结果贵州省县级公立医院医务人员职业风险总体得分为(2. 69±0. 97)分,属于"比较不满意";单因素分析结果显示,医务人员不同文化程度(F=12. 767,P <0. 001)、职称(F=9. 300,P <0. 001)、职业类型(F=34. 325,P <0. 001)和工龄(F=6. 080,P <0. 001)的职业风险比较,差异有统计学意义;多元逐步回归分析结果显示,年龄(t=3. 588,P <0. 001)、文化程度(t=-6. 240,P <0. 001)、职业类型(t=7. 301,P <0. 001)、工龄(t=-5. 816,P <0. 001)、领导与管理(t=9. 142,P <0. 001)、培训与晋升(t=8. 263,P <0. 001)、工作条件(t=13. 890,P <0. 001)、人际关系(t=-4. 182,P <0. 001)和报酬(t=11. 479,P <0. 001)对医务人员职业风险有影响。结论医务人员职业风险得分低于一般水平,应落实医疗责任保险制度,分散医务人员职业风险与责任;重视职业风险管理教育,提高各类医务人员职业风险防范意识;优化职业风险影响因素,为医务人员构建和谐的职业环境。
OBJECTIVE Job satisfaction directly affects the enthusiasm of medical staff.The purpose of this study is to investigate the satisfaction level of occupational risk of medical staff during the reform of county-level public hospitals in Guizhou Province,explore its influencing factors,and provide a scientific basis for relevant departments to formulate policies to mobilize the enthusiasm of medical staff.METHODS From September 5,2016to March 8,2017,2532 medical workers in the aggregate from 7county-public hospitals in Guizhou were selected by using cluster sampling for survey on the occupational risk.Single factor analysis and multiple stepwise regression analysis were used to analyze the influencing factors of occupational risk.RESULTS The satisfaction level of occupational risk of medical staff in county-public hospitals was not quite satisfied as a whole.There were statistically significant differences between different medical staff with various factors such as level of education (F=12.767,P<0.001),professional title(F=9.300,P<0.001),trpe of occupation(F=34.325,P<0.001)and work experience(F=6.080,P<0.001).Additionally,there were a lot of factors affected occupational risk of medical staff,including age(t=3.588,P<0.001),degree of culture(t=-6.240,P<0.001), kinds of occupation(t =7.301,P<0.001),working-age (t=-5.816,P<0.001),leadership and management(t =9.142, P<0.001),training and promotion(t=8.263,P<0.001),working conditions(t=13.890,P<0.001),interpersonal relationship(t=-4.182,P<0.001),and remuneration(t=11.479,P<0.001).CONCLUSIONS The survey reveals that satisfaction level of occupational risk of medical staff is lower than the general one.To improving job satisfaction level,we should implement the medical liability insurance system to spread occupational risks and responsibilities of medical staff,pay attention to increase occupational risk management to improve the awareness of occupational risk prevention for all kinds of medical staff and optimize occupational risk factors to construct dispersal mechanism of the occupational risk to build a harmonious occupational environment for medical staff.
