摘要
目的探讨白球蛋白比值(AGR)与系统性红斑狼疮(SLE)病情活动的关系。方法收集确诊的301例SLE患者,根据SLE疾病活动度(SLEDAI)对患者疾病活动程度进行评分,将SLEDAI <10分记为病情稳定组(n=65),将SLEDAI≥10分记为病情活动组(n=236)。回顾性分析患者的临床资料、实验室检查,采用单因素分析、二分类logistic回归分析、受试者工作特征(ROC)曲线探讨SLE病情活动与AGR的关系。结果病情稳定组与病情活动组间比较红细胞沉降率(ESR)、淋巴细胞计数、血红蛋白、血小板计数、天门冬氨酸氨基转移酶、血清白蛋白(Alb)、AGR、尿素氮、肌酐、尿酸、抗核抗体、抗ds-DNA抗体、补体(C_3、C_4、CH_(50))、24 h尿蛋白定量差异有统计学意义(P <0. 05)。调整混杂因素后发现C_3降低、血小板减少、AGR降低及24 h尿蛋白定量升高是SLE病情活动的独立危险因素(P <0. 05)。AGR与SLE病情活动的ROC曲线下面积(AUC)为0. 665(P <0. 000 1)。结论血清AGR水平降低可能是评估SLE病情活动的简单有效指标。
Objective To investigate the correlation between albumin to globulin ratio( AGR) with disease activity in systemic lupus erythematosus( SLE). Methods A total of 301 patients with SLE were selected. The clinical data and lab indexes were retrospectively analyzed. Binary logistic regression analysis and ROC curve of serum AGR level were used to investigate the correlation of AGR with disease activity. Results According to SLEDAI,inactive group( n = 65,SLEDAI = 0-9) and active group( n = 236,SLEDAI≥10) were allocated. The results of univariate analysis showed that erythrocyte sedimentation rate,lymphocyte count,hemoglobin,platelet count,aspartate aminotransferase,Alb,AGR,urea nitrogen,creatinine,uric acid,antinuclear antibody,antideoxyribonucleic acid antibody,complements( C3,C4,CH50) and 24 h urinary protein were significantly different between inactive and active group( P < 0. 05). Binary logistic regression analysis found that C3,platelet count,AGR and 24 h urinary protein quantitative were significantly associated with disease activity in SLE patients( P < 0. 05). The area under the ROC curve( AUC) of AGR was 0. 655( P < 0. 000 1).Conclusion The decreased serum AGR level may be a simple and useful biomarker for evaluating the disease activity in SLE patients.
