摘要
目的:探讨茵陈越鞠汤联合多烯磷脂胆碱对非酒精性脂肪性肝病的治疗作用及对肝纤维化指标的影响。方法:选取98例非酒精性脂肪肝患者作为研究对象,并按照随机数字表法将其分为对照组和试验组。对照组48例患者在采用常规护肝治疗的同时,静脉滴注多烯磷脂酰胆碱注射液,每天1次;试验组50例患者在对照组治疗方案的基础上加服茵陈越鞠汤,4周为1个疗程。治疗后,观察两组患者的临床疗效、中医症状、体征评分,并对比分析患者的肝功能指标,包括谷草转氨酶(aspartate aminotransferas,AST)、谷丙转氨酶(alanine aminotransferase,ALT)、谷氨酰转肽酶(valley ammonia acyl turn peptidase,GGT)以及肝纤维化指标,如层黏蛋白(laminin,LN)、透明质酸钠(through Ming sodium hyaluronate,HA)、Ⅲ型前胶原(typeⅢprocollagen,C-Ⅲ)和Ⅳ型胶原(typeⅣcollagen,C-Ⅳ)水平的变化情况。结果:经过4周治疗,两组患者的临床症状均得到明显改善,其中试验组显效率为94. 00%,明显高于对照组的77. 08%,差异有统计学意义(P <0. 05)。治疗后,两组患者食欲不振、恶心、肝脾肿胀、疼痛的中医症状、体征评分较治疗前均显著降低(P <0. 05),且试验组患者降低幅度更大,两组比较,差异有统计学意义(P <0. 05)。治疗后,两组患者的肝功能和肝纤维化各指标水平较治疗前显著改善(P <0. 05),且试验组患者改善程度更为明显,两组比较,差异有统计学意义(P <0. 05)。结论:茵陈越鞠汤联合多烯磷脂胆碱治疗非酒精性脂肪肝效果较好,不但能够有效缓解患者的临床症状,改善机体肝功能,而且具有增强患者抗纤维化的作用。
Objective:To investigate the therapeutic effect of Yinchen Yueyu Decoction combined with polyene phosphatidylcholine on nonalcoholic fatty liver disease and its influence on liver fibrosis index.Methods :98 patients with nonalcoholic fatty liver disease were selected as subjects and divided into control group and experimental group according to random number table method.In the control group,48 patients were treated with conventional liver protection,and intravenous polyene phosphatidylcholine injection was given once a day.In the experimental group,50 patients were treated with Yinchen Yueju Decoction in addition to that of the control group.4 weeks was considered as one course of treatment.After treatment,the clinical efficacy,TCM symptom scores of the two groups were observed,and the liver function indexes of the patients were compared and analyzed,including aspartate amin-otransferas (AST),alanine aminotransferase (ALT),and glutamyltranspeptidase (valley).Ammonia acyl turn peptidase GGT) and indicators of liver fibrosis,such as laminin (Lin),sodium hyaluronate (through Ming sodium hyaluronate HA),type Ⅲ procollagen C-Ⅲ,and type Ⅳ collagen (Type Ⅳ collagen C-Ⅳ)changes in levels.Results :After 4weeks of treatment,the clinical symptoms of both groups were significantly improved.The effective rate of the experimental group was 94.00%,which was significantly higher than that (77.08%)of the control group (P <0.05).After treatment,the scores of TCM symptoms and signs of loss of appetite,nausea,hepatosplenomegaly and pain in the two groups were significantly lower than those before treatment (P<0.05),and the decrease in the test group was greater with academic significance (P<0.05).After treatment,the levels of liver function and liver fibrosis in the two groups were significantly improved compared with those before treatment (P<0.05),and the improvement in the test group was more obvious.The difference between the two groups was statistically significant (P<0.05). Conclusion :Yinchen Yueyu Decoction combined with polyene phosphatidylcholine is effective in treatment of nonalcoholic fatty liver disease.It can not only effectively relieve the clinical symptoms of patients,improve the liver function of the body,but also enhance the anti-fibrosis effect of patients.
