摘要
目的确定妊娠期高血压疾病妇女产后终末期肾病的长期风险。方法回顾性分析比较浙江大学舟山医院2016年6月至2018年6月中1 363名具有妊娠期高血压疾病的女性和3 111名正常对照的女性,评估终末期肾病的发病情况及其临床特征,并针对潜在的干扰因素使用调整的Cox回归分析确定终末期肾病的相对风险。结果在1 363名妊娠期高血压疾病患者中,16名(1.17%)患有终末期肾病。有妊娠期高血压疾病的女性患有终末期肾病的风险比无妊娠期高血压疾病的女性高10.64倍(95%CI:7.53～15.05)。有先兆子痫病史合并慢性高血压的女性风险最高(OR=66.95,95%CI:34.36～130.44)。有妊娠期高血压疾病的女性患终末期肾病的风险高于无妊娠期高血压疾病的女性(OR=7.85,95%CI:2.92～21.10)。结论无论其患有哪种类型的高血压疾病,妊娠期高血压疾病患者产后终末期肾病的风险较高,因此建议有妊娠期高血压病史的妇女定期进行产后检查,尤其是肾功能检查。
Objective To determine the long-term risk of postpartum end-stage renal disease in women with hypertensive disorder complicating pregnancy.Methods We retrospectively analyzed 1363 women with hypertensive disorder complicating pregnancy and 3111 normal controls from June 2016 to June 2018in Zhoushan Hospital of Zhejiang University to evaluate the incidence and clinical features of end-stage renal disease,and to determine the relative risk of end-stage renal disease by using adjusted Cox regression analysis for potential interfering factors.Results Of 1363 patients with hypertensive disorder complicating pregnancy,16 women suffered end-stage renal disease.Cases of hypertensive disorder complicating pregnancy had a 10.64-fold risk of end-stage renal disease as those without gestational hypertension (95%CI:7.53-15.05).Women with history of pre-eelampsia complicating chronic hypertension had the highest risk (OR =66.95,95%CI:34.36-130.44).Women with hypertensive disorder complicating pregnancy had a higher risk of end-stage renal disease than women without gestational hypertension (OR=7.85,95%CI:2.92-21.10).Conclusion Regardless of the type of hypertensive disorder,the risk of postpartum end-stage renal disease is higher in patients with hypertensive disorder in pregnancy.We recommend that women with history of hypertension in pregnancy should take regular postpartum examinations,especially renal function test.
