摘要
目的分析焦作市卫生资源配置均衡性与公平性,提出优化配置的对策建议。方法运用HRDI比较国内一线城市、省会城市与焦作市2010-2016年卫生资源配置均衡性;运用基尼系数与泰尔指数分析焦作市卫生资源配置公平性及变化趋势。结果一线城市(北京、上海、广州、深圳)与省会城市(郑州)在卫生资源配置方面优于焦作市;按人口/按地理面积配置的床位数、医生、护士三项指标基尼系数总体呈现下降趋势,2016年分别为0. 2777/0. 4799、0. 2166/0. 3893、0. 3839/0. 5723;泰尔指数降低情况,床位数由0. 0917降至0. 0614,医生由0. 0492降至0. 0388,护士由0. 1226降至0. 1157。结论焦作市2010-2016年卫生资源按人口配置公平性优于按地理面积配置公平性,护士的配置公平性最差,其次为床位数和医生。卫生资源配置不公平的主要原因是区域间的不公平。
Objective To analyze the proportionality and fairness of health resources allocation in Jiaozuo so as to put forward some countermeasures and suggestions for the optimization. Methods Using HRDI to compare the proportionality of health resources in first-line cities,the provincial capital cities and Jiaozuo during 2010 and 2016. Gini coefficient and the Theil index were used to analyze the fairness and change trend of health resources allocation in Jiaozuo. Results The firstline cities( Beijing,Shanghai,Guangzhou,Shenzhen) and the provincial capital city( Zhengzhou) were superior to Jiaozuo in the allocation of health resources. The Gini coefficient according to demography/geography of the three indicators of bed,doctor and nurse showed a downward trend in general,in 2016 the data were as follows: 0. 2777/0. 4799,0. 2166/0. 3893 and 0. 3839/0. 5723;the Theil index of three indicators dropped from 0. 0917,0. 0492,0. 1226 to 0. 0614,0. 0388,0. 1157 during 2010 and 2016. Conclusion The fairness of health resources allocation in Jiaozuo according to demography is better than that according to geography,nurse allocation is the worst of the three indicators,followed by bed numbers and doctors.The main reason for unfairness in health resources allocation is the inequity between regions.
