传统腹腔镜与机器人辅助腹腔镜技术在儿童肾盂成形术中的临床应用对比 被引量:17

Clinical analysis of conventional laparoscopic and robotic-assisted laparoscopic pyeloplasty in children

摘要 目的比较在儿童传统腹腔镜肾盂成形术(LP)与机器人辅助腹腔镜下肾盂成形术(RALP)的手术效果及适用范围。方法回顾性分析2014年1月至2016年5月间,应用传统腹腔镜或机器人辅助腹腔镜技术进行单侧离断式肾盂成形术的儿童病例资料。结果共计39例单侧肾盂输尿管连接部狭窄(UPJO)患儿,其中24例接受LP,男20例,女4例;15例接受RALP手术治疗,男14例,女1例。LP和UPJO两组年龄分别为(3.2±1.1)岁和(7.5±3.4)岁,手术时间分别为(167±25)min和(173±31)min,平均随访时间分别为22.3个月和17.5个月,平均出血量均为15ml。所有手术均未中转开放手术,术后住院期间均无短期并发症。两组均有1例患儿术后肾积水加重继而进一步手术治疗。结论机器人辅助腹腔镜肾盂成形术可有效、安全地应用于3岁以上的UPJO患儿,并能取得与传统腹腔镜手术类似的效果。 Objective To compare the effect and scope of conventional laparoscopic and robotic-assisted laparoscopic pyeloplasty in children. Methods A retrospective study was performed in children undergoing conventional laparoscopy and robotic assisted laparoscopic pyeloplasty from January 2014 to May 2016. All patients underwent dismembered pyeloplasty (Anderson-Hynes). Results A total of 39 patients of ureteropelvic junction obstruction (UPJO) were enrolled,. There were 20 boys and 4 girls. And laparoscopy (LP, 14 boys versus 1 girl) and robotic surgery (RALP, 14 boys versus 1 girl) were performed. The mean age was 3.2 (LP) vs. 7.5 (RALP) years, mean operative duration was 167 (LP) vs. 173 (RALP) min, the mean estimated blood loss 15 ml and the mean follow-up period 22.3 (LP) vs. 17.5 (RALP) months. The procedure was performed without any conversion into open surgery. There was no onset of perioperative complications. There was recurrence in each group. Conclusions Robotic assisted laparoscopic pyeloplasty may be safely performed in children aged over 3 years with a similar efficacy to that of traditional laparoscopy.
