摘要
目的探讨血清铁蛋白(serum ferritin,SF)水平对多发性骨髓瘤(multiple myeloma,MM)诊断的临床意义。方法选择2016-01至2018-06在同济大学附属第十人民医院血液科初治的32例MM患者和39例健康志愿者(对照组)作为研究对象,MM组患者已排除引起铁蛋白升高的其他因素。比较MM组与对照组SF表达水平。通过ROC曲线检测SF在MM患者中的诊断价值,随访32例MM组患者每次化疗前的SF水平以及相应的β2-微球蛋白(β2-microglobulin,β2-MG)水平及乳酸脱氢酶(lactic dehydrogenase,LDH)水平共计169人次,并对检测结果进行统计分析。结果 MM组SF(546.60±427.66) ng/ml明显高于对照组(231.20±111.70) ng/ml,差异有统计学意义(t=4.06,P=0.000)。ROC曲线分析中,SF水平曲线下面积为0.75(P=0.000),具有一定的诊断意义。随访32例MM患者共169人次,定期化疗前SF与β2-MG有正相关关系(r=0.714,P<0.01),而与LDH水平无明显相关关系(r=0.059,P> 0.01)。Ⅲ期MM患者中SF水平明显高于Ⅰ期和Ⅱ期,差异具有统计学意义(P<0.05)。结论 SF对MM有辅助诊断价值,能反映一定程度的肿瘤负荷,SF结合β2-MG对MM患者分期和预后判断有重要价值。
Objective To analyze the clinical significance of serum ferritin(SF) in patients who were diagnosed with multiple myeloma(MM).Methods Thirty-two newly-diagnosed MM patients treated in the Department of Hematology of Shanghai Tenth Hospital between January 2016 and June 2018 were enrolled in the MM group,while another 39 healthy volunteers were assigned to the normal control group in the same period of time.Other factors that could increase levels of SF were excluded in the MM group.The levels of SF of the two groups were detected to find out whether there was statistically significant difference between the two groups.The diagnostic value of SF in MM patients was determined by the ROC curve.SF levels and related levels of β2-microglobulin(β2-MG)and lactate dehydrogenase(LDH) before each cycle of chemotherapy in the MM group were followed up totally for 169 person-times,and statistical analysis was performed.The value of SF at different stages of MM patients was further analyzed.Results The levels of SF in the 32 patients with MM(546.60±427.66) ng/ml were significantly higher than those of normal controls(231.20±111.70)ng/ml(P=0.000).In the ROC curve analysis,the area under the SF level curve was 0.75(P=0.000),which was of diagnostic significance.According to the levels of SF,β2-MG and LDH in MM patients before each cycle of chemotherapy,it was found that there was a positive correlation between SF levels and β2-MG levels(r=0.714,P<0.01),but no significant correlation with LDH levels(r=0.059,P>0.05).SF levels in stage Ⅲ MM patients were significantly higher than those in stage Ⅰ and stage Ⅱ MM patients(P<0.05).Conclusions SF is of diagnostic value for MM,which can reflect tumor burden to some extent.SF combined with β2-MG can facilitate staging and prognosis of multiple myeloma.
