摘要
目的磁共振平衡全稳态快速场梯度回波(B-FEE)与三维时间飞跃法(3D—TOF)序列联合应用对原发性三叉神经痛(TN)患者进行颅脑扫描,分析TN患者血管神经压迫(NVC)的影像学特点,评价磁共振对三叉神经痛的诊断价值。方法将51例单侧原发性TN患者纳入研究,所有患者颅脑两侧均完成B.FFE与3D—TOF序列扫描,层厚1mm,在两序列及相关后处理图像上分析患者症状侧和非症状侧NVC程度和位置。结果TN患者NVC程度,症状侧与非症状侧差异有统计学意义(P<0.05)。两组症状侧出现NVC者D值差异有统计学意义(P<0.05)。其中,两组近者D值差异有统计学意义(P<0.05);中者D值差异无统计学意义(P>O.05);远者D值差异无统计学意义(P>0.05)。结论磁共振B.FFE与3D.TOF序列联合应用对NVc中重度压迫显示良好,对临床提供术前评估和指导治疗有极其重要的意义。
Objective To evaluate the diagnostic value of MRI for trigeminal neuronal compression(NVC)in patients with primary trigeminal neuralgia(TN)by brain scan combined with B-FEE and 3D-TOF sequence.Methods A total of 51 patients with unilateral primary TN were included in the study.B-FEE and 3d-TOF sequence scans were performed on both sides of the skull,with thickness of 1ram.The NVC degree and location in symptom side and non-symptom side were analyzed on two sequences and postprocessing image.Results The NVC degree in TN patients showed significant differences between two sides(P<0.05).The D value in NVC in symptom side were significantly different between two groups(P<0.05).The D value is significantly different for short distance between two sides(P<0.05),while no significant differences were found for medium and large distance between two sides(P>0.05).Conclusion The combined use of MRI B-FEE and 3D-TOF sequences shows good effect on moderate and severe compression in NVC,and is of great significance in providing preoperative evaluation and guidance for clinical treatment.
