摘要
目的分析孕中期孕妇铁营养状况与甲状腺功能的相关性。方法常规产检的孕中期孕妇442例分为三组:铁缺乏(ID)组142例,血清铁蛋白<15μg/L,Hb>110g/L;缺铁性贫血(IDA)组148例,血清铁蛋白<15μg/L,Hb<110g/L;正常对照(C)组152例,血清铁蛋白≥15μg/L,Hb≥110g/L。检测并比较三组TSH、FT_3、FT_4、甲状腺过氧化物酶抗体(TPOAb)和尿碘水平。结果 C组、ID组和IDA组尿碘中位数分别为200、150和150μg/L,差异无统计学意义(P>0.05)。IDA组血清FT_3和FT4水平低于ID组和C组[(4.13±0.37)pmol/L vs.(4.20±0.51)pmol/L和(4.49±0.46)pmol/L和(13.64±1.78)pmol/L vs.(13.90±1.70)pmol/L和(15.33±1.85)pmol/L](P<0.05),且ID组血清FT_3及FT_4水平亦低于C组(P<0.05)。Pearson相关性分析显示,血清铁蛋白与FT_3和FT_4呈正相关(r=0.376和0.336,P<0.01)。结论孕中期孕妇营养性ID与甲状腺功能的改变密切相关。
Objective To analyze the correlation of iron nutrition status and thyroid function in pregnant women of second trimester.Methods A total of 442 pregnant women in the second trimester was divided into three groups.The patients in group ID(142 cases)were with serum ferritin<15μg/L and Hb>110 g/L,those in group IDA(148 cases)were with serum ferritin<15μg/L and Hb<110 g/L and those in group C(152 cases)were with serum ferritin≥15μg/L and Hb≥110 g/L.Serum levels of thyroid stimulating hormone(TSH),free triiodothyronine(FT3),free thyroxine(FT4)and thyroid peroxidase antibody(TPOAb)as well as urinary iodine were detected and compared among three groups.Results The median urinary iodine in groups of C,ID and IDA were 200,150,150μg/L,respectively,which was not significantly different among three groups(P>0.05).Serum FT3 and FT4 levels in group IDA were lower than those in groups of ID and C[(4.13±0.37)pmol/L vs.(4.20±0.51)pmol/L and(4.49±0.46)pmol/L and(13.64±1.78)pmol/L vs.(13.90±1.70)pmol/L and(15.33±1.85)pmol/L](P<0.05),which were lower in group ID than those in group C(P<0.05).Pearson correlation analysis showed that serum ferritin was positively correlated with FT3 and FT4(r=0.376 and 0.336,P<0.01).Conclusion The nutritional iron deficiency is closely related to thyroid function in pregnant women in the second trimester.
