摘要
目的比较在乳腺外科的临床教学中,利用乳房触诊模拟器教学,相对传统的利用真实患者教学,对学生学习效果的影响。方法以2013级及2014级蚌埠医学院第一附属医院肿瘤外科见习的临床本科学生为研究对象。2013级临床见习生(对照组n=40)以门诊和住院真实乳腺病患者进行乳房体格检查教学,2014级临床见习生(实验组n=64)以乳房触诊模拟器进行乳房体格检查教学。均在教学完成后2个月的学期考核中对教学效果进行考核,并对教学过程进行问卷调查,对所有结果进行统计分析。结果在腋窝淋巴结触诊、乳房触诊、乳头溢液检查测试中,实验组成绩优于对照组(χ~2=4.42,P=0.04;χ~2=4.78,P=0.03;χ~2=7.72,P=0.01);在检查过程中与患者适当交流互动测试中,对照组成绩优于实验组(χ~2=6.86,P=0.01);实验组学习热情和主动学习频率要高于对照组(χ~2=23.58,P=0.01);学生对应用乳房触诊模拟器教学的满意度更高(H=6.873,P=0.01);两组的理论考试的结果无显著差异(t=0.92,P=0.36)。结论在乳腺外科的临床教学中,利用乳房触诊模拟器教学能提高教师的教学效率及学生的满意度。
Objective To compare the effect of using mamma simulators or real patients in the clinical teaching of breast surgery.Methods The undergraduate students of grade 2013 and 2014,those who learn on the job in the first affiliated hospital of Bengbu Medical College were studied.In the teaching of breast physical examination,outpatient or inpatient patients with breast diseases were adopted in the grade of 2013( control group,n = 40),and mamma simulators were used in the class of 2014( experimental group,n = 64).The two groups of students completed the final examination within 2 months after the clinical teaching,questionnaire survey about the teaching process was conducted,and the examination results were statistically analyzed.Results In the examination of axillary lymph node palpation,breast palpation and nipple discharge,the results of the experimental group were better than that of the control group( χ^2= 4.42,P = 0.04;χ^2= 4.78,P = 0.03;χ^2= 7.72,P= 0.01).In the test of proper communication and interaction with the patients during the examination,the results of the control group were better than that of the experimental group( χ^2= 6.86,P = 0.01).The active learning frequency of the experimental group was higher than that of the control group( χ^2= 23.58,P = 0.01).Students ’learning satisfaction with the using of mamma simulators was higher( H = 6.873,P = 0.01).There was no significant difference in the results of theoretical examination between the two groups( t = 0.92,P=0.36).Conclusions In the clinical teaching of breast surgery,using of mamma simulators could improve the teaching efficiency of teachers and the satisfaction of students.
