MIPPO联合LCP对胫腓骨骨折患者骨折愈合、关节功能的影响分析 被引量:2

摘要 目的:探讨微创经皮钢板内固定(MIPPO)联合锁定加压钢板(LCP)对胫腓骨骨折患者骨折愈合时间、关节功能的影响。方法:选择胫腓骨骨折患者120例,按随机数表法分为微创组与传统组,每组各60例。传统组采用切开复位内固定治疗,微创组采用MIPPO结合LCP治疗,两组患者术后均随访10个月。统计两组患者围术期指标,通过Baird-Jackson踝关节评分、Rasmussen膝关节评分标准评估踝、膝关节功能,统计两组患者术后膝关节活动度及术后发生并发症情况。结果:微创组术中出血量、手术时间、住院时间、骨折愈合时间、切口长度均低于传统组(P<0.05);术后3个月、10个月,微创组Baird-Jackson踝关节、Rasmussen膝关节评分均高于传统组(P<0.05);术后10个月,微创组伸膝、屈膝活动度均高于传统组(P<0.05);微创组发生术后并发症发生率低于传统组(P <0.05)。结论:MIPPO联合LCP治疗胫腓骨骨折能有效缩短骨折愈合时间并改善关节功能。 Objective:To explore the effect of minimally invasive percutaneous plate internal fixation(MIPPO) combined with locking compression plate(LCP) on fracture healing and joint function in patients with tibial fracture.Methods:120 patients with tibiofibular fractures were enrolled.The patients were divided into minimally invasive group and traditional group according to the random number table method,60 cases in each group.The traditional group was treated with open reduction and internal fixation.The minimally invasive group was treated with MIPPO combined with LCP.Both groups were followed up for 10 months.The perioperative indexes of the two groups were statistically analyzed.The performance of the knee joint was evaluated by the Baird-Jackson ankle joint score and the Rasmussen knee joint score.The postoperative knee joint activity was counted.The proportion of postoperative complications in the two groups was counted.Results:The amount of bleeding,operation time,hospitalization time,fracture healing time and length of incision in the minimally invasive group were lower than those in the traditional group(P<0.05).After 3 months and 10 months,in the minimally invasive group,Baird-Jackson ankle joint and Rasmussen knee joint scores were higher than the traditional group(P<0.05).At 10 months after operation,the knee and knee flexion activities of the minimally invasive group were higher than the traditional group,and the difference was statistically significant(P<0.05).The proportion of postoperative complications in the minimally invasive group was lower than that in the traditional group(P<0.05).Conclusion:MIPPO combined with LCP in the treatment of tibiofibular fractures can effectively shorten the fracture healing time and improve joint function.
