摘要
目的评价本院普外择期手术住院患者营养状况。方法普外科1002例择期手术住院患者,男430例,女572例;年龄15~89岁;消化道内疾病411例(良性181例,恶性230例),消化道外疾病591例(良性506例,恶性85例)。入院后3天对所有患者进行多项营养指标检测:(1)人体测量与理想体重比(WT/HT),上臂皮褶厚度(TSF),上臂肌围(AMC),握力(GS);(2)实验室检查血红蛋白(HB),血浆白蛋白(ALB),运铁蛋白(TF),肌酐身高指数(CHI),淋巴细胞总数(TLC);(3)生物电阻抗法测定人体脂肪(FAT)。结果各项指标表明营养不良的发生率分别为:WT/HT18.1%,TSF25.8%,AMC22.2%,GS65.5%,HB51.2%,ALB6.1%,TF24.9%,CHI47.6%,TLC23.7%,FAT63.6%。各项指标全部正常者占12.5%,1~2项指标异常者占46.9%,3~5项指标异常者占37.7%,6~8项指标异常者占2.9%。结论患者住院时即存在不同程度的营养不良,积极进行围手术期营养支持非常必要。
Objective To evaluate the nutritional status of1002selective general surgical patients of our hospital.Methods We treated1002(430M,572F)selective surgical cases(aged15to89)wit hin15months in general surgery of our hospital.411out of1002cases had digestive tract diseases(benign181,malignant 230)and591had nondigestive tract disease(benign506malignant 85).A lot of nutrition indexes were examined in all patients within three days after hospitalization.Above mentioned indexes include:(1)anthropometry:WT /HT,TSF,AMC and GS;(2)laboratory examin-ation:HB,ALB,TF,CHI and TCL;(3)measuring body fat with bioelectricity impedance method.Results The incidence of abnormal nutrition indexes:WT /HT(18.1%),TSF(25.8%),AMC(22.2%),GS(65.5%),HB(51.2%),ALB(6.1%),TF(24.9%),CHI (47.6%),TLC(23.7%),FAT(63.6%).Among these patients,12.5%had all normal parameters,41.3%had3~5abnormal,and11.4%had6~8abnormal.Conclusions The nutrition assessment of1002cases indicates the certain amount of patients were malnorished before hospitalization.It is necessary to launch perioperative nutritional support.
