老年男性2型糖尿病合并高血压患者血清同型半胱氨酸水平变化与肥胖、胰岛素抵抗的关联性研究 被引量:12

摘要 目的探讨在老年男性2型糖尿病合并高血压患者中血清同型半胱氨酸水平的变化。方法试验分3组:2型糖尿病组(22例)、2型糖尿病合并高血压组(31例)、健康对照组(26例)。空腹检测各组静脉同型半胱氨酸、血糖、胰岛素、维生素B12、瘦素、总胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白、载脂蛋白(a)等指标,测量血压,同时测量并计算各组的腰围/臀围、体重指数和胰岛素敏感指数。结果2型糖尿病合并高血压组的同型半胱氨酸、瘦素、收缩压、舒张压、BMI均高于其他两组,ISI、叶酸、维生素B12均低于其他两组。多元线性相关分析显示,同型半胱氨酸与收缩压、舒张压、瘦素、BMI呈正相关,与叶酸、维生素B12、ISI呈负相关,以上比较皆达到统计学意义。结论在老年男性2型糖尿病合并高血压的患者中,更易发生同型半胱氨酸水平的增高,这种关系可能与肥胖和胰岛素抵抗之间存在一定的关联。 Objective To explore homocysteine(Hcy)level in old males with type 2diabete s accompanying hypertension.Methods The cohort was divided to three groups,22cases of 2-DM,31cases of 2-DM with h ypertension and 26normal controls.Plasma homocysteine,blood glucose,plasma insulin,lept in,VitB 12 ,folate,lipid were measured in these groups.Meanwhile,blood pressure,WHR,BMI and ISI were measured and calculated.Results The level of Hcy,leptin,sBP,dBP,BM I in the group of 2-DM with hypertension were obviously higher than those in other two groups,and ISI,VitB 12 ,folate were lower.Bivariate corre lation analysis show,Hcy was signif icantly correlated with sBP,dBP,BMI,leptin positively and with VitB 12 ,folate and ISI negatively.Conclusions Hcy level in old males with type 2diab etes accompanying hypertension was high er than that of 2-DM and normal subjects,possible which was correlated wi th obesity and insulin resis-tance.
