摘要
目的探讨胰腺实体假乳头肿瘤(solid pseudopapillary tumor,SPT)的病理特点。方法 12例SPT患者的手术标本均用组织学(HE和PAS)、免疫组化染色(Envision法)方法研究。结果 光镜下,实性区肿瘤细胞呈假乳头状或片状排列,假乳头中心或肿瘤间质内有PAS阳性的粉染小体。免疫组化显示,肿瘤内α1-AT、α1-ACT阳性细胞呈弥漫分布 (100%),Vimentin阳性细胞呈散在分布(83.3%),NSE和CHG在肿瘤细胞内呈局灶性阳性(分别为16.7%和33.3%),PCNA2例呈阳性(16.7%),CK8阳性细胞局灶分布(8.3%),Insulin例呈阳性(8.3%),ER均为阴性。结论 胰腺实体假乳头肿瘤多发生于年轻女性,检测ER表达均阴性,未显示肿瘤和雌激素有关系;肿瘤生长缓慢可能和瘤细胞多处于非增殖期状态有关;肿瘤可能是胰源性的,部分有向神经内分泌细胞发展的可能。
To study the clinicopathologic characteristics of solid pseudopapillary tumors (SPT) of the pancreas. Methods Twelve patients with SPT were studied using histologic (HE and PAS) and immunohistochemical techniques (Envision method). Results These tumors exhibited solid sheets with a pseudopapillary pattern. PAS- positive globules were at the stalks of the pseudopa-pilles or in the mesenchyme of the tumor. Immunoreactivity for α1 - AT and α1 - ACT was detected in 12 tumor tissue samples (100% ). Ten samples(83. 3%) showed immunostain for Vimentin, 2 samples (16.7%) for NSE, 4 samples(33.3%) for CHG, and 2 samples (16. 7%) for PCNA.Only one sample (8.3%)showed immunostain for CK8 and Insulin. ER protein was negative. Conclusions SPT occurs predominantly in adolescent girls and young women. But negativity for ER can't show correlation between the tumor and estrogen. Slow growth of the tumor relates to the unhyperplastic stage of the tumor cells. The immunohistochemical results point to the possibility of the origin of the tumor from pancreas, and differentiation of the tumor cells to nerf or endocrine cells.
