摘要
目的 观察通窍活血汤对家犬蛛网膜下腔出血(SAH)后脑血管痉挛的防治作用。方法 治疗组与对照组家犬各9只,分别予通窍活血汤和尼莫地平片经胃造瘘管连续给药10d,给药第3天、6天采用“二次枕大池注血法”复制SAH模型,造影测量a-首次注血前30min、b-首次注血后30min、c-首次注血后第7天基底动脉(BA)口径。结果 治疗组中:b(0.64mm±0.10mm)与a (1.27mm±0.04mm)比较P<0.01,c(1.22mm±0.07mm)与a比较P>0.05,首次注血后30min(b/a约50.56%)BA严重痉挛、第7天无痉挛。对照组:b(1.24mm±0.06mm)与a(1.29mm±0.05mm)比较P>0.05,c(0.84mm±0.15mm)与a比较P<0.01,首次注血后30min无痉挛、第7天(c/a约65.12%)严重痉挛。第7天家犬昏迷计分治疗组明显高于对照组(P<0.05)。结论通窍活血汤对SAH后早发性脑血管痉挛防治效果差,而对迟发性脑血管痉挛防治效果明显。
Objective To observe the effects of Tongqiaohuoxue Decoction (TD) on the cerebral vasospasm after subarachnoid hemorrhage (SAH) in canines. Methods Eighteen canines were randomly divided equally into TD group and control group, in which the animals were given Nimodipine. SAH model were established by injecting the autologous blood into the cisterna magna. The diameters of the basilar arteries (BA) were meseaured by digital subtraction angiography. Results In TD group, the diameter of the basilar artery ( 0.64mm±0.10mm) at 30 min following the first injection of the autologous blood was significantly smaller than that (1.27mm±0.04mm) at 30 min before the first injection of the blood (P<0.01), which was significantly different from that (1.22mm±0.07mm) on day 6 following the first injection of the blood (P<0.05). In the control group, the diameter of the basilar artery (1.24mm±0.06mm) at 30 min after the first injection of the blood was insignificantly different from that (1.29mm ±0.05mm) at 30 min before the first injection of the blood (P>0.05), which was significantly bigger than that (0.84mm±0.15mm) on day 7 after the first injection of the blood (P<0.01). Conclusions TD has insignificant preventive effect on early cerebral vasopasm, but significant preventive effect on delayed cerebral vasopasm after SAH in the canines.
